General Information Document Type: Sources Sought Notice Reference Number: W91YTZ-15-T-0118 Current Response Date: 29 May 2015 Classification Code: 65-Medical, Dental, and Veterinary Equipment and Supplies NAICS Code: 325413-Chemical Manufacturing--In-Vitro Diagnostic Substance Manufacturing Contracting Office Address The North Regional Contracting Office (NRCO), Northern Regional Medical Command (NRMC), 6011 5th Street, Bldg. 1469, Ft Belvoir, VA 22060 The North Regional Contracting Office (NRCO) is issuing this sources sought notice as a means of conducting market research to identify parties having an interest in and the resources to provide the following: 1. SCOPE OF WORK: The manufacturer shall provide the attached list (and below) medical materiel and supplies as per specification detailed in the itemized contract listing. Materials are to be provided to the United States Army Fort George G. Meade, Department of the Army, Kimbrough Ambulatory Care Center (KACC), Fort Meade, MD 20755. 2. DELIVERY LOCATION: All deliveries of procured medical materiel and supplies from the manufacturer placed under this contract are to be delivered to the Logistics Division, BLDG 2484 (Warehouse), USAMEDDAC Fort Meade and Kimbrough Ambulatory Care Center, 85th Medical Battalion Avenue, Fort George G. Meade, Maryland 20755. 3. GENERAL REQUIREMENTS: A. Contractor/Manufacturer: 1. The contractor/manufacturer must be approved and licensed by the FDA and have no current outstanding violations with respect to appropriate FDA procedural and regulatory guidelines or policies. 2. The contractor/manufacturer's facility will be compliant with USP regulations and recommended USP 797 guidelines. Manufacturer USP 797 adherence policies and procedures shall be made available to KACC upon request. Issues and concerns involving USP 797 guidelines will be addressed promptly. 3. The contractor shall supply the medical materiel and supplies as specified with respect to specific size, dimensions, and estimated annual KACC usage requirements as annotated in the attached listing. Variations in amounts and requirements will fluctuate with actual patient requirements and surgeries. KACC will have no obligation to procure unnecessary items. KACC will execute a minimum order of $3,000.00 worth of supplies for this contract. B. KACC OR Medical Materiel Specifications: 1. 100% Item Listing Support - the manufacturer must be able to supply all of the items on the attached item listing in the required size, dimensions, and volume of estimated annual requirements. Any deviation from the requirements specified size and dimensions cannot be used to substitute for the required itemized list. The attached volumes are not guaranteed as requirements will vary yearly in representation of the patients and procedures required by KACC. 2. The additional surgical materials listed must be available for procurement by KACC. Manufacturer must be able to supply more than the estimated annual amounts as specified in the detailed itemized contract listing due to the variation associated with patient procedures; the listing provides an estimate of the annual requirements of each item. Price listing/quote for 100% of the products sizes and options must be supplied by the vendor to support all size and requirements for hand, wrist, elbow, and foot surgeries even though all items and sizes are not listed on the itemized contract listing. C. KACC Materiel Availability: 1. KACC must be notified immediately as to the availability of any of the products listed on the itemized contract listing. Items that are no longer available or discontinued must be provided so as not to necessitate a cut back or cancellation of patient's surgeries or procedures. 2. KACC is authorized to buy additional materiel from the manufacturer as required by mission until the threshold of the contract amount has been reached. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. After results of this market research are obtained NRCO may conduct a competitive procurement and subsequently award a Contract. If at least two responsible, qualified respondents are classified as small business described herein, then any competitive procurement solicited by NRCO will be done as a small business set-aside. NRCO is seeking responses from all responsible sources, including small businesses. Companies that can provide the equipment are requested to email a detailed report describing their abilities to yvonne.t.parson.civ@mail.mil no later than the response date for this sources sought notice. The report should include achievable qualifications and any other information relevant to your service or capabilities. Also, the following information is requested to be provided as part of the response to this sources sought notice: 1. Name of the company, POC name, phone number, address and email address; 2. NAICS code; 3. Descibe the experience your company has in managing similar program(s). 4.
