- QASP Wage Determination Performance Work Statement Clauses Price Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 1. Solicitation number F3P4C15076AW01 is issued as a Request for Quotation (RFQ). 2. This solicitation document and incorporated provisions and/or clauses are those in effect through Federal Acquisition Circular (FAC) 2005-82. 3. The North American Industry Classification System (NAICS) Code is 812320, Dry-cleaning and Laundry Services, with a Small Business size standard of $5,500,000.00. This acquisition is Unrestricted i.e. not set-aside. 4. Contract Line Item Numbers (CLINs) 0101, 0201, 0301, 0401, and 0501 will apply to this requirement. The Quantity and Unit of Issue is 1 Lot, and Lots will consist of the items defined and priced at Attachment 4. The Period of Performance is a total of five years with a potential extension of up to six months. 5. Description of Requirement: Non-Personal Services to provide all supervision, labor, materials, equipment, and transportation (pick-up and delivery) necessary to perform Medical Laundry Services for the 359th Medical Treatment Facility located on Joint Base San Antonio - Randolph, in accordance with the Performance Work Statement (PWS) dated 28 May 2015 at Attachment 2, Price Schedule at Attachment 4 and the terms and conditions of this RFQ and resultant contract. 6. The following provisions/clauses are applicable to this RFQ and included in full text at Attachment 1: 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offerors Representations - Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Deviation). Note: Throughout this document and its attachments, the terms "offer" and "offeror" are understood to mean "quote" and "quoter". 52.212-4 Contract Terms and Conditions - Commercial Items is incorporated by reference however see Attachment 1 for a full text addendum to the clause. Other provisions and/or clauses applicable to this requirement are also contained in Attachment 1. 7. The Defense Priorities and Allocations System (DPAS) rating program does not apply to this requirement. 8. The time and date for receipt of quotes is not later than 2:00PM Central, 29 June 2015. Submit offers to the Contract Specialist, Vivian Fisher at vivian.fisher@us.af.mil with a copy to the Contracting Officer, Radames Gandia at radames.gandia.us.af.mil. Quotes may also be mailed to 502 CONS/JBKCA/Attn: Vivian Fisher/395 B Street West, Suite 2/Joint Base San Antonio-Randolph 78150 or hand-delivered (however, quoters are cautioned of potential delays that may be encountered during base access). E-mail is the preferred method of transmission. Quotes received after the date and time specified will be considered "late" and may not be eligible for award consideration. Quotes are to remain valid for a period of not less than 90 days. 9. This RFQ is NOT to be construed as a commitment by the United States Air Force. The Government is NOT liable for any costs and/or activity associated with providing a response to this announcement. Any costs incurred as a result of a response to this announcement shall be borne by the firm and will not be charged to the government for reimbursement. 10. To be eligible to receive an award resulting from this RFQ, contractors must have an active registration in the System for Award Management (SAM) database, no exceptions.
