Description

The purpose of this amendment is to address the Questions received in response to Solicitation Number: VA249-15-Q-0509 - Services and to provide an Updated Statement of Work. Please see attached. 1.Question: Are control cubes required for sampling above ceiling tiles in patient care areas? Answer: Yes 2.Question: Can you please provide a little more information on the extent of basements, crawlspaces, roof tops, what floors are considered patient areas and if you have a mobile containment system that can be used for infectious control if we'll be in patient areas. Answer: Refer to schematic plans attached. All floors have patient areas. VA Memphis does not have a mobile containment unit available. Answer: Schematic plans attached. 4.Question: Please confirm that only friable ACM is required to be labelled. For instance VAMC's surveyed in the past have not wanted floor tiles and other non-friables with ACM to be labelled. Answer: The areas must be identified but not labeled. However, it is required to be added into our O & M Plan. 5.Question: Please clarify how to provide the pricing information i.e. firm fixed fee for entire project except for lab fee for bulk sample analysis and separate line items for unit pricing for PLC and PCM sample analysis? Answer: Pricing should be firm fixed and should not be broken down into separate line items.
