Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Responses are due to the Government point of contact listed below, no later than 12:00pm PST on Friday, June 5th 2015. This RFQ is for a Firm Fixed Price contract. The Government intends to award a contract to a qualified Small Business offeror deemed responsive in accordance with the Federal Acquisition Regulation (FAR) Part 12, whose quote conforms to all the RFQ's requirements and is judged to represent the best value to the Government in accordance with the evaluation scheme set forth in this RFQ. Evaluation will be based upon the Lowest Price Technically Acceptable (LPTA) and delivery of requirements will be FOB destination. This office will consider any late quotes or any late revisions of quotes as non-responsive. In no event shall any understanding or agreement between the offeror and any Government employee other than the Contracting Officer on any contract, modification, change order, letter, or verbal direction to the offeror be effective or binding upon the Government. All such actions must be formalized by a proper contractual document executed by an appointed Contracting Officer. The offeror is hereby notified that in the event a Government employee other than the Contracting Officer directs a change in the work to be performed or increases the scope of the work to be performed, it is the offeror's responsibility to make inquiry of the Contracting Officer before deviating from the written terms of the contract. Payments or partial payments will not be made without prior authorization by an appointed Contracting Officer with the legal authority to bind the Government. The North American Industry Classification System (NAICS) code for this acquisition is 532490. Any changes must be coordinated through the Government point of contact and approved in writing by the Contracting Officer. This acquisition is under Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures. This requirement is for: Mobile Containerized Shower & Sink Unit Rental CLIN 0010 12-STALL SHOWER TRAILER RENTAL x 1ea Period of Performance shall be June 14-24, 2015 See below Statement of Work for specific requirements Vendor must quote quote mark all or nothing quote mark and must have an active registration in System for Award Management (SAM) prior to award. Important information: Any FAR references can be found in full text at http://farsite.hill.af.mil/ Company information must be registered in SAM (see www.SAM.gov ) STATEMENT OF WORK 1.0 DESCRIPTION OF SERVICES. The Washington Army National Guard (WAARNG) 81ST HBCT requires a mobile containerized shower and sink unit. The contractor will perform the services outlined in this SOW. 2.0 BACKGROUND. This SOW outlines requirements for providing mobile containerized shower and sink unit for the Washington Army National Guard. 3.0 REQUIREMENTS. The contractor will provide transportation, 24 hour maintenance, power generation, clean water delivery and storage and grey water disposal during Annual Training in a field environment. The Government requires the contractor to deliver, set up and pick-up the mobile containerized shower and sink unit to the unit's training location in the vicinity of the Yakima Training Center. Specific Location will be given upon award of the contract. 3.1 The contractor will provide a mobile containerized shower and sink unit that contains twelve (12) shower heads and 12 (12) sink basins. The mobile containerized shower and sink unit will be used from 14 - 24 June 2015. 3.2 The contractor will provide all necessary equipment to complete the assigned mission. The shower/sink unit will be utilized to provide showers and personal hygiene for up to 90 Soldiers every day for 10 days. It is estimated that shower/sink unit will utilize approximately 1,500 gallons of hot and cold water per day and 1,500 gallons of grey water per day. 3.3 The contractor will provide equipment necessary to complete the emplacement of the mobile containerized shower and sink unit. 3.4 The contractor will ensure placement of the mobile containerized shower and sink unit are coordinated with unit point of contact and are delivered on time and that drivers have proper identification and pass badges. 3.5 The contractor will provide a project manager representative responsible for the conduct and completion of the contract. The project manager shall be proficient in speaking and writing in the English language. The project manager will interface with the unit and will ensure timeliness of delivery and pick-up. The project manager will provide the unit with all the necessary contact information prior to drop-off of container. 3.6 The contractor will provide a clean water holding tank for up to 1,500 gallons and a grey water tank for up to 1500 gallons. The contractor will be responsible for providing a clean water solution for the unit as well as any grey water disposal. Contractor will provide a sealed wastewater collection system and waste removal trucks. Disposal of wastewater will be at an approved discharge point. 3.7 Units are to have hot and cold water. 1500 gallon of hot water will be provided at all showering times. Units are to have individual stalls, with changing stations. Units will have climate control, interior lighting and exhaust fans. Units will be constructed of non porous surfaces and non slip flooring. 4.0 PERIOD OF PERFORMANCE. Period of performance is 14-24 June 2015. Showers will be available for use 0500-1100 and 1300-2300 daily. Cleaning will occur daily from 1100-1300 daily. 5.0 SERVICING. The contractor must be available 24 hours a day, 7 days a week by phone in case of emergency maintenance. The contractor must be able to service or replace the mobile containerized shower and sink unit within 24 hours if unit becomes non functional after 24 hours. 5.1 The contractor is responsible for ensuring the clean water tank emptied. 6.0. GENERAL INFORMATION. 6.1 Badging/Screening. The contractor will have the proper credentials to deliver the mobile containerized shower and sink unit to the training site. It will be the contractor's responsibility to ensure that the vehicle and driver(s) are able to enter a military installation. 6.2 Worksite Safety. The contractor shall be responsible for safety of employees during all operations in accordance with the Army and OSHA safety regulations and guidance. 6.3 Medical Care. The contractor is responsible for all medical care for employees. 6.4 Restricted Areas. The military installation is intended for authorized personnel only. The contractor will be limited to the dirt/unpaved surface to and from the main entrance at YTC and to the delivery site.
