Description

This is a Sources Sought Notice. This is NOT a solicitation for proposals, proposals abstract, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified sources; (2) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. The NAICS code is 333314 and the Size Standard is 500. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a small business set-aside is possible. Specifically, Tripler Army Medical Center (TAMC) is seeking qualified sources who can provide an Zeiss Cirrus HD-OCT 5000 Tomographer. The tomographer shall be compatible with the existing Government-owned equipment, Zeiss Cirrus HD-OCT 4000. All qualified vendors are invited to submit capability statements that substantiate their experience and qualifications in providing subject above within five (5) days from the date of this notice. Interested vendors shall indicate if they have provided similar tomographer to military medical treatment facilities and/or the general public. Responses to this notice should include company name, address, and telephone number, point of contact (POC), and complete product details, brochures, and etc. which will allow the Government to understand your offerings and options. Capability statements should be submitted to the attention of Ms. Linda Oshiro, Contract Specialist, Pacific Regional Contract Office, 160 Krukowski Road, Tripler, HI 96859. Telephone responses will not be accepted. Responses must be received no later than 5 June 2015 at 10:00 a.m. Hawaiian Standard Time. Disclaimer and Important Notes. The Government reserves the right to use information provided by respondents for any purposes deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organizations qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. No proprietary, classified, confidential, or sensitive information should be included in your response.
