- This is a REQUEST FOR INFORMATION notice for market research purposes only to determine the availability of qualified small business sources capable of providing the full range of services described below. Responses should be provided via email to stephanie.keo@va.gov no later than 3:00pm central time on June 5, 2015. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information to be considered sensitive or proprietary. Respondents will not be notified of the results of the market research. The appropriate NAICS Code 622110 (General Medical and Surgical Hospitals) and the small business size standard $38.5M. The Michael E. DeBakey VA Medical Center (MEDVAMC), 2002 Holcombe Blvd, Houston, TX 77030 is seeking a leader in the field of both Ventricular Assist Device (VAD) and heart transplants to refer our MEDVAMC veterans to for care. We are seeking only institutions or providers with a lifetime experience of greater than one hundred VAD implantations and forty or more heart transplants or greater than forty VAD implants and ten heart transplants in 2014. The Contractor shall provide Heart Transplantation and Ventricular Assist Device (VAD) services to include: Left Ventricular Assist Device (LVAD); Right Ventricular Assist Device (RVAD); Bi-Ventricular Assist Device (BVAD); and Total Artificial Heart (TAH) services, for the Government, to Veterans Affairs (VA) beneficiaries referred by the VA National Transplant Program and VA Medical Centers across the country that cannot be accommodated by a Government in-house transplant program for reasons such as, but not limited to, scheduling, capacity, or waiting lists. The Government may refer local patients for transplantation under this Agreement. The hospital that performs these procedures should be in close proximity (10 miles) to the MEDVAMC to be able to accommodate on sites visits by MEDVAMC physicians and nurses that may be necessary to access the patient's status. Interested capable sources to provide: 1) Company name, address, point of contact, phone number, e-mail address, and DUNS number. 2) SAM registration and/or ORCA as evidence of size status. 3) Company holds a GSA/FSS contract, if any and provides the contract number 4) Brief capability statement (no more than 5 pages) with enough information to clearly demonstrate the capability to provide the full range of required services, and information on past performance providing relevant services.
