- Description
- The Department of Veteran Affairs, Southern VA Health Care System (SAVAHCS) in Tucson, Arizona ("VA") requires a contract to provide waste water analysis for the PVAHCS in Phoenix, AZ. This will be a full service contract with option years for continued service and any emergency support. The VA anticipates issuing a solicitation for this requirement on May 29, 2015 with a closing date for proposal June 9th, 2015. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for download copies of solicitation (and any subsequent amendments) from that website. No hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they notified of the issuance of the solicitation and any amendments. NAICS code 541620 with a small business size standard of $15 Million. The solicitation will be issued as a 100% Service-Disabled Small Business Set-Aside. All responsible sources may submit proposals which shall be considered by the Agency It is anticipated that only one award will be made for the waste water analysis services at PVAHCS. See Below for detailed description of requirement: D.1 SPECIAL CONTRACT REQUIREMENTS 1.SERVICE: Waste Water Analysis for PVAHCS a.The contractor will provide all qualified personnel (labor), expertise, tools, travel expenses, transportation, and necessary parts to perform a comprehensive preventive maintenance (PM) and repair service (if necessary) for the equipment and systems listed below for Phoenix Arizona VA Health Care System (PVAHCS). b.The contractor will perform waste water sampling, monitoring to include the package and transport with chain of custody, record, and send the completed wastewater (effluent) analysis report to the Phoenix VA Health Care System (PVAHSC), Engineering Department (139), 650 E. Indian School Road, Phoenix, AZ 85012-1892. All work shall be performed in accordance with the City of Phoenix Wastewater Discharge Permit #1201-2590 (attached) for the Phoenix VA Health Care System (PVAHSC) using the designated City Self-Monitoring Report forms (sample attached). c.GENERAL REQUIREMENTS i.Site Visit: If a site visit is deemed necessary arrangements may be made. In the case of a site-visit, Contractors are strongly encouraged to attend site visit prior to preparation of offer. ii.Test Laboratory Certification: All Contractors must utilize laboratories certified by Arizona State laws for accreditation of environmental testing laboratories to ensure the quality of analysis data used for regulatory purposes. During the term of the contract, the Contractor shall be required to provide a valid certificate number and provide an updated certificate and list of approved fields of testing for the laboratory subcontracted for sample analysis. All Arizona state and Environmental Protection Agency (EPA) sampling and analysis methods and standards applicable shall form a part of this statement of work. If a subcontractor is being used to perform the sampling analysis, the above information also applies to that subcontractor. The VA reserves the right to inspect the laboratory used upon request. iii.Standards: Whenever two or more standards apply in a given situation, the most stringent standard shall govern. iv.Field Testing: Field-testing instruments may be used where indicated provided that: (1) the operator is trained in proper use, calibration, and maintenance of the instrument, (2) the testing instruments shall be calibrated and serviced in accordance with manufacturer's recommendations, and (3) field calibration reports are provided (using Phoenix ESD forms) along with appropriate test results. d.DELIVERIES AND PERFORMANCE i.Analysis and Testing: ii.The Contractor shall perform necessary sampling during the weekday within the first fifteen (15) calendar days of the month. The Contractor shall notify the designated Contracting Officer Representative (COR) of intended sampling date 24 hours in advance. iii.Analysis shall be in conformance with the City of Phoenix Wastewater Discharge Permit #1201-2590 (copy attached) for the Phoenix VA Health Care System (PVAHCS). Permit #1201-2590 with the concurrent tests expires on December 31, 2016. During the Option Year in which this permit is renewed for PVAHCS, should no changes occur in the sampling requirements, no changes in the contract pricing will be approved; however, should changes in sampling requirements occur as a result of the permit requirements, an equitable adjustment will be negotiated. iv.At the time of this contract base period, Total Petroleum Hydrocarbon (TPH) sampling is temporarily suspended and shall not be included in the Contractor monthly pricing. Should TPH sampling be reinstated as a requirement under the terms of this contract agreement, the Contractor shall have the right to request an equitable adjustment in the monthly contracting pricing for this additional test. v.All three Parshall Flumes (2590.04 - 6 Inch; 2590.05- 3 Inch; and 2590.6 - 3 Inch North) shall be inspected for cleanliness and conditions that may affect proper orientation. Flumes will be cleaned on a monthly basis. Structural deficiencies detected will be reported to the designated Contracting Officers Representative (COR) for recommended repairs. No repair work is expressly authorized under the terms of this contract agreement and shall be negotiated as separate items. All three Parshall Flumes (Sampling Points) are confined spaces. Consequently 29 CFR 1910.146 applies. Appendix E, Sewer System Entry provides specific information appropriate to this issue. (See attached). vi.Report Submittals: Reports shall be submitted to the Phoenix VA Health Care System (PVAHCS), Engineering (139), by the 20th of the month following in which the sampling was performed in the Significant Industrial User Monitoring Report as provided. vii.Temporary Increased Sampling Methods (TISM): This is a firm fixed price contract with monthly fee non-inclusive of all monthly, quarterly, and TISM tests that may be required to conform to the City of Phoenix Wastewater Discharge Permit for the Phoenix VA Health Care System (PVAHCS) when permit parameters are exceeded. No TISM sampling is expressly authorized in the terms of this agreement and shall be negotiated as separate items as required. e.The Contractor shall be fully responsible for compliance with all VA, local, state, and Federal environmental/occupational safety laws, rules and regulations. The Contractor shall follow life and safety codes, and take necessary actions to avoid conditions that may be hazardous to the health and safety of hospital personnel and patients. The Contractor is responsible for meeting all applicable OSHA and VA guidelines for safe working processes. All work for the duration of the contract shall be performed by certified trained technicians. 2.EQUIPMENT: Three Parshall Flumes shall have appropriate flow-measuring devices installed by PVAHCS. Contractor will not need to provide flow-measuring equipment for the Parshall Flumes. a.Location: To be serviced on site PVAHCS (Phoenix, AZ). 3.PERFORMANCE: a.Service will be scheduled with a contact person as assigned by the Contracting Officer's Representative (COR), Point of Contact (POC) or designee for performance on site. Before doing any work, the service representative will report to Mr. Robert Carlson (Bldg. 28 at PVAHCS) to log in and obtain a contractor identification badge, sign a log maintained by the COR and after the work is done, sign out. This log will state the nature of the visit and the time and place of the visit. The COR can be contacted at (602) 277-5551 ext. 6335. b.Failure to carry out the above procedures will be considered as a failure to document the service visit and will constitute a failure to perform. c.Normal duty hours are 8:00 a.m. through 4:30 p.m., Monday through Friday, not including federal holidays. Whenever contact with PVAHCS is required outside of normal duty hours, the Utility System Repair Operator (USRO) may be contacted as an alternate. The hospital operator at extension "0" can contact the USRO operator via radio. d.At the completion of the service call, the service representative will submit a complete service report, as described in section 5 - Documentation, to COR or POC or designee. e.Service will be scheduled during the timeframe of the contract year. Prior to scheduled month of service, the Contractor will provide the COR a schedule of the weekdays this service is to be performed, for approval. The COR shall have the authority to reschedule this service within twenty-four (24) hours of scheduled time due to emergency and/or prior weather situations. Contractor shall inspect, adjust, maintain, repair, and test equipment identified in accordance with OEM specifications. Any parts used shall be OEM. f.The contractor is responsible for supplying all materials necessary to complete any of the service work. g.The contractor is to contact the COR Mr. Robert Carlson if any additional service fees may need to be applied. The COR can be contacted at (602) 277-5551 ext. 6335. h.Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. Example: wearing of distinctive clothing such as uniform, badges, patches, etc. i.The Contractor shall provide all tools, testing equipment for disassembly, assembly, adjustment, calibrating and other work as required for the performance of this contract. j.The Contractor shall be responsible for cleaning the work area and equipment upon completion of work. All accumulated rubbish shall be removed. 4.OVER AND ABOVE/EMERGENCY REAIR: a.Upon discovery of a malfunctioning instrument, the Contractor shall immediately notify the COR or POC staff onsite. Any repair job shall not be initiated until approval is obtained from the Contracting Officer via a modification to the contract. Charges for repair, labor, and parts shall originate only when required and upon prior approval being granted by the VA. At the VA's option the Contractor may be requested to provide a price quotation for the repair job. If the contractor has deemed the repair is outside the scope of the contract, the contractor should notify the Contracting Officer in writing the nature of the repair within 24 hours. 5.DOCUMENTATION: a.Service reports will be provided for all service activities including preventive maintenance, emergency repair, and safety inspections, and must contain the following information: i.Date(s) and time period of service. ii.Complete description of equipment serviced including model number and serial number. iii.Complete description of equipment services performed including upgrades, updates and software changes made. iv.Complete list of all materials, parts, or assemblies installed or replaced including serial numbers of parts where applicable. v.A statement of the complaint that initiated the service call. vi.Total standard labor hours, total overtime/premium hours, and total travel hours expended during the repair. 6.REMOVAL OF EQUIPMENT: a.Whenever possible, all repairs and preventive maintenance will be performed at the PVAHCS. When that is not possible, the COR, POC or designee may give approval for service work to be done at another location. All transportation charges will be borne by the contractor. b.VA property may not be removed from the Medical Facility prior to execution, and approval of all documents as required by Acquisition and Materiel Management Service. Computer hard drives and other storage media items may not leave VA property. c.Requirements for service reports for designated equipment not at this location are the same as for work done on-site. Service reports for incoming equipment will be presented for signature at the time of delivery. If equipment is returned via commercial carrier, the service report may be included with the shipment. 7.RESPONSE TO SERVICE REQUESTS: a.The contractor must respond to requests for emergency service in the following manner: i.A competent service representative adequately equipped with tools, test instruments, and repair parts will arrive at the Medical Facility within the designated response time. ii.The competent service representative will be prepared to begin work immediately to resolve the complaint as expressed by the COR, POC or designee when service was requested. Work needed to resolve the complaint will continue without interruption until the problem is resolved. iii.The service representative will come equipped with all the service/technical manuals needed to perform the work. iv.The service representative may contact VA staff by phone before arrival at the facility. Such contacts, however, will not serve to alter the requirements for arrival on-site except when the request for service is withdrawn by Biomedical Engineering. v.The designated response time for this contract is 24 hours from the time of the request for service. If repeated requests are made before the arrival of contractor's repairer, the first request will determine the required time of arrival. A.All requests for repair service will be considered requests for emergency service unless a statement to the contrary is made by the COR, POC or designee. Such statements will be documented on the service report. 8.GOVERNMENT REPAIRS: The government is not obligated to perform any work on contract equipment. 9.GUARANTEE: The contractor will guarantee workmanship and all parts furnished by the contractor for a period of not less than commercial market guarantee provided to non-governmental entities. 10.INFORMATION SECURITY: The equipment does not contain any patient sensitive information, nor is it connected to the VA network. The equipment does not gather or store sensitive data; therefore, there is no sensitive data to protect (Handbook 6500.6, Appendix A, Question 7). The C&A requirements do not apply, and a security accreditation package is not required.
