- Wage Determination 05-2047 Performance Work Statement (PWS) This is a combined synopsis/solicitation issued as a SMALL BUSINESS SET-ASIDE for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number F1STAH5139B001 is issued as a Request for Quotations (RFQ) and is conducted as a commercial item procurement using Simplified Acquisition Procedures (FAR Part 13) for Non-Personal Service (Performance-Based Service Acquisition to install Three Thermoplastic Signs and Remove Existing Surface Paint see Attach Performance Work Statement (PWS) for more information). Interested vendor must be Airmark Certified Installer (Thermoplastic Sign manufacturer). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC), Number 2005-82 and DPN: 20150420. The associated North American Industry Classification System (NAICS) code is 238210. Size standard is $15 million. This RFQ has one contract line item number (CLIN) for Non-Personal Services: See attached PWS. The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to install three Thermoplastic signs and remove existing surface paint as stated in the PWS. Quotations shall be accompanied by a completed Representations and Certifications (Reference: FAR 52.212-3, dated Mar 2015). The Government's preferred method is electronic Representations and Certifications. This can be accomplished by registering in SAM at https://sam.gov and completing the on-line Representations and Certifications. Contract financing is NOT provided for this acquisition. Quotation must identify payment terms. The Government intends to award firm fixed price (FFP) purchase order of non-personal service resulting from this solicitation to the responsible offeror(s) whose quote is the lowest price and passing past performance result. The Government intends to award without discussions and the Government reserves the right not to make an award at all. By submitting a quote, the offeror acknowledges the requirement that potential awardees must be registered in the System Award Management (SAM) prior to award which can be accomplished at https://www.sam.gov. Quotations may be in any format with following information: proposing company's name, address and phone; point of contact's name, phone, email address, quotation date; quotation number; total price; identify any discounts; how long the quote is valid (minimum of 60 days). Quoted price shall be FOB DESTINATION. Delivery location of service is at 412 TW Air Force Plant 42, Palmdale CA 93550. The following Federal Acquisition Regulation (FAR) provisions apply to this acquisition: 52.212-1, Instructions to Offerors - Commercial Items (Apr 2014) 52.212-2, Evaluation Factors (Oct 2014) [The evaluation factors for the fill-in portion of this provision will be handled in accordance with FAR 13.106-1 and 13.106-2. Evaluation criteria will be lowest price and past performance. Past performance criteria will be passing or fail base on Past Performance Information (PPI) collected.
