- Existing Handholes and Manholes to Bldg 346 Example of Patch Panel Layout TTIA-EIA-569-A-7-Cable Tray CAT6 Cable Color Code Q & A Bldg 346 CAT 6 Cabling List of Materials for the Review B346 OSP CVC Snap Shot This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation FA6648-15-T-0003 is being issued as a Request For Quotation (RFQ). This solicitation document with the incorporated provisions and documents are those in effect through Federal Acquisition Circular 2005-72. The Department of the Air Force 482nd Homestead ARB, FL has a requirement for the installation of new cable tray on the first and second floor of Building 346, new premise wiring, new racks, re-route Outside Plant (OSP) copper and SM fiber, removal of existing outdated premise wiring in Building 346 at Homestead ARB, FL. It also defines the requirement for the Contractor to install and test as well as provide construction support for the NIPRNet premise wiring in Building 346 IAW with the attached Statement of Work (SOW) dated 13 April 2015 for Premise Wiring for Building 346. Period of performance is contingent upon award date. A site visit of the area for service is scheduled for 10:00 A.M. on 19 May 2015. Provide the name(s) of the representative(s) from your company who will be attending by noon on 15 May 2015 to Antonio.Jones.13@us.af.mil. Attendance at the site visit is encouraged. There will not be an opportunity to schedule an alternate day for a site visit if your company cannot attend. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The North American Industry Classification System (NAICS) Code for this requirement is 811213 and small business size standard is $11 M. A fixed price contract will be awarded for this solicitation. Offerors are required to be registered in Systems for Awards Management (SAM) to receive an award. The solicitation will be made available only on FedBizOps website. Paper copies will not be available. No telephone calls or written requests for this solicitation package will be accepted. Once the solicitation is posted, it is the interested parties' responsibility to review the site frequently for any updates/amendments to the solicitation. Proposals are due in the Homestead ARB, FL contracting office no later than noon local time on 5 June 2015. Proposals must be sent via email to Antonio.Jones.13@ us.af.mil. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Such inquiries shall be directed to the Contracting Officer. 5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (Nov 2012) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and N/A to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with N/A citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment.
