- ATTACHMENT 4: PRICING TABLE FORM ATTACHMENT 3: PAST PERFORMANCE QUESTIONNARE FORM ATTACHMENT 2: CDC SCHEDULE & SOW ATTACHMENT 1: COMBINED SYNOPSIS INFO Combined Synopsis/Solicitation for Clinical Care for Individuals Referred by CDC for Medical Examination and/or Hospitalization i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (see applicable informational attachments.) ii. The solicitation number is 2015-N-17184 and is being issued as a request for proposal. iii. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73. iv. This acquisition is for full and open competition. The North American Industry Classification System (NAICS) code is 621498 with a size standard not to exceed $10.0 million (U.S.) v. Information concerning the contract line item numbers and items, quantities and units of measure including options are included within Attachment 2 under Section B. vi. A brief description of the CDC requirement is as follows: CDC seeks a qualified contractor to provide a mechanism for transportation, initial consultation, examination, isolation, diagnosis and treatment of CDC referrals as appropriate based on the medical nature of the referral. Workplace injuries are to be treated according to existing clinical guidelines and hospital practice. In the case of suspected exposure (includes persons under investigation or evaluation (PUI) and/or showing signs of disease due to serious communicable infectious agents or agents of unknown origin), asymptomatic and symptomatic individuals may require immediate isolation, evaluation, treatment or other medical care under this contract. See Attachment 2, the Statement of Work (SOW) for more detail. vii. Performance is to begin July 14, 2015 and continue through July 13, 2016 with four 4 twelve month option periods. Past Performance form can be found under attachment 3 and the PricingTable under attachment 4. viii. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. See Attachment 1 for Addenda to the provision. ix. The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. See Attachment 1, for Addenda to the provision that includes evaluation factors and evaluation criteria. x. Offerors shall include a completed copy of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. (Note, if offerors are registered within the System for Award Management (SAM) and certifications and representations are up to date, make a statement to that affect in accordance with the provision at Attachment 2, pg. 26) xi. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. xii. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial items, applies to this acquisition and FAR clauses applicable are 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-40, 52.222-54, 52.225-13, 52.232-33, and 52.222-51. Other applicable clauses including Health & Human Services Acquisition Regulation (HHSAR), and CDC applicable clauses can be found within Attachment 1. xiii. There are no special contract requirements or terms and conditions related to contract financing arrangements or warranty. xiv. The Defense Priorities and Allocations System does not apply to this acquisition. xv.
