Description
Vendor Classification Vendor Questionnaire W-9 Terms and Conditions I. Project Scope of Work Provide design and construction to update and/or modify the existing surveillance system into one central surveillance system with the utilization of a web-based application. The center presently uses a surveillance system that is outdated and needs repair in order to meet desired coverage of the Center. Several cameras have failed and those will need to be replaced or modified to meet desired coverage. The newly designed system must include a digital recorder to back-up all cameras with DVR storage. Provide color screen monitoring in the Gate House. Head and equipment will be located in the Administration Building 10 in IT closet. The project should include the replacement of all non-working existing cameras and new site wiring installed to existing code. Contractor are to include necessary training to staff in the proper operation of the equipment. Warranty information should be included in the bid package. Additional maintenance or service contracts (i.e. one-year, three-year, five year) must be presented in the contractor' response but will be negotiated separately at the time of the award. Minority-owned and women-owned businesses are encouraged to apply. II. General Conditions: Long Beach Job Corps Center will host a pre-bid conference at: Long Beach Job Corps Center, June 12, 2015 at 9:00 AM. Note: The pre-bid conference is not mandatory, but highly encouraged. Contractors who wish to attend the pre-bid conference must call no later than June 11, 2015 by 12:00 Noon to register your attendance. Any questions must be submitted/emailed to the Buyer by June 19, 2015. Q&A will be emailed to all respondents on June 22, 2015. No work may be started without the issuance of a purchase order. Bid proposal to include breakout of material, labor and mark-up. Contractor must be familiar with the components of a surveillance system. All work to be code compliant and installed in a professional manner. All work on the center must comply with OSHA standards. All contractors working at Long Beach Job Corps Center are required to sign in and out each day. All contractors are subject to the Long Beach Job Corps Center rules and regulations for work at this site. Vehicles entering and exiting the center are subject to search. Work may be scheduled between the hours of 8:00 AM - 3:30 PM, Monday - Friday. There should be no un-necessary interaction between contractor and the Long Beach Job Corps Center students. Contractors must dress appropriately and visitor's badges must be worn. Contractors must comply with the Davis/Bacon Act (prevailing wage). The respondent must also include a position descriptions and rate of pay for each individual conducting work on the center. Wage determination must be in accordance with prevailing wage for the Long Beach area. All forms must be filled out correctly or the proposal may be considered as non-responsive. Bids are due by June 19, 2015 by 12:00 Noon. Submit bids in writing to: Long Beach Job Corps Center, Attn: Lisa Markouizos, Buyer; 1903 Santa Fe Ave., Long Beach, CA 90810. Note: The Buyer is designated as Odle Management's representative for this procurement. III. Tentative Schedule of Events RFP Release June 1, 2015 Bidders Conference June 12, 2015 Proposal Deadline (not subject to change) June 19, 2015 by 12:00 pm (noon) Approval and Awarded by June 23, 2015 Award of contract of June 23, 2015 Anticipated contract Start Date July 1, 2015 III. Notice of Award All respondents will be notified by mail as to their award status. Unsuccessful respondents who wish to obtain information on the evaluation of their proposal should submit a written request to: Attn: Lisa Markouizos, Buyer; 1903 Santa Fe Ave., Long Beach, CA 90810. ( IV. Cost and Negotiations Attachments The Partnership reserves the right to reject any or all proposals received and to negotiate with any and all offers on modifications to proposals. Bids submitted which are over the maximum amount of funds specified for this RFP will be rejected. The proposal warrants that the costs quoted for services in response to the RFP are not in excess of those that would be charged any other individual for the same services performed by the bidder.
