Description
The Department of Veterans Affairs awarded a firm fixed priced contract to procure the Next Generation (1527 channel) 2-Dimensional real time 2 dimensional real time diode with PC software interface with I year of hardware and software maintenance from Sun Nuclear, 3275 Suntree Blvd, Melbourne, FL. 32940 to be integrated with the same equipment previously purchased to support Radiation Therapy. The total value of this order shall not exceed $49,149.00 JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3K) BUT NOT EXCEEDING THE SAT ($150K)) IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. This template when completed can be used to document single source awards IAW FAR13.106-3(b)(3). Note: Statements such as "only known source" or "only source which can meet the required delivery date" are inadequate to support a sole source purchase. 1. PURCHASE REQUEST OR REQUISITION NUMBER: 523-15-3-7821-01671A. PROJECT/TASK NUMBER1B. ESTIMATED AMOUNT: $49,403.00 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: MapCHeck 2 Next Generation (1527 channel) 2-Dimensional real time diode array. MapPHAN -2 (5cm) Water equivalent phantom securely holds MapCHECK ® 2 array at measurement plane depth of 5cm and backscatter depth of 5cm. Measurement may occur in the coronal and sagittal positions 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: The Radiation Therapy Department currently utilizes the MapCheck system for the Quality Assurance of each patients treatment. It is necessary to assure that each plan created for each individual patient is accurate and with in 2% of the Dr.'s prescribed dose. With this updated version we will be able to continue to QA many of the new advancements in treatment being performed here at the VA. Also, the MapPHAN is necessary to QA the MapCheck device making sure it is within the necessary specs to accurately QA patients data. 4. REASON THAT SUGGESTED SOURCE IS THE ONLY SOURCE, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: The Radiation Therapy Service has been utilizing the MapCHeck system for many years. There is a solid program set up used to maintain safe and accurate treatments to our veterans. Changing to a different QA system would cause a delay in treatments due to the need to start over a QA program with new devices and establish new data, knowledge and training. 5. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: The Physics department with in the Radiation Therapy Service has been utilizing this QA system for many years. They obtained the quote for the system. This QA system is necessary to continue the QA program set up here at the Boston VA Medical Center and maintain a high quality of treatment for each veteran. 6. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
