- FAR 8 - JUSTIFICATION FOR LIMITING SOURCES <$150K Requirement or RFQ Number: 610-15-3-279-0069 1.Description of Action: Nature: 1 New Requirement 0 Follow-on Order 0 Mod to Existing Order Number Order against: 1 FSS Contract Number: V797P-4444B Name of Proposed Contractor:Medline Industries, Incorporated Street Address:One Medline Place City, State, Zip:Mudelein, IL 60060-4486 Phone:(847) 949-5500 or (800) 533-5469 2.Description of Supplies or Services: The estimated value of the proposed action is $38,190.96 The Northern Indiana VA Health Care System has submitted a request to purchase four (4) MBSD10700 - Scanner, Bladder, Biocon 700, with printer for the facility. This compact, portable 3-D device provides quick, safe and non-invasive measurements of bladder volume and post-void urinary retention. This particular product offers real-time pre-scan that assists in improving accuracy, reduces improper scans and saves time by viewing the bladder before performing the test. The built-in printer facilitates immediate documentation in the patients chart. Additional features of this product include: various modes for child, male or female, voice recording, high storage capacity, and durability, encapsulated and protected probe. This item is used with a combination of other equipment currently in operation at the facility and is a required component. This item appears on the FSS Contract V7974P-4444 - Medline Industries, Inc. is the only vendor that sells this product on the Schedule 65. 3.Authority: ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception: 0 An urgent and compelling need exists, and following the procedures would result in unacceptable delays(FAR 8.405-6(a)(1)(i)(A); 1 Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; (FAR 8.405-6(a)(1)(i)(B)) or 0 In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures. (FAR 8.405-6(a)(1)(i)(C) 4.Rational/explanation to support cited authority: This item is unique and is a necessary component that is used in patient care. This particular product offers real-time pre-scan that assists in improving accuracy, reduces improper scans and saves time by viewing the bladder before performing the test. The staff at the Northern Indiana Healthcare System has received extensive training on this device and exporting patient information that is then entered into the patient's file. To purchase a new device after the extensive training would delay patient care and require the Northern Indiana Healthcare System to use 2 different scanning systems. This would not only require additional training while learning how to use the new device but the variation that may exist in the extraction of patient data from the scanner of 2 different systems may jeopardized patient care. The Northern Indiana Healthcare System currently has four (4) devices that are shared between two CBOC's and the Ft. Wayne and Marion facilities. The purchase of the additional four (4) devices will be distributed accordingly: two (2) devices going to each CBOC locations in Indiana, one (1) device is for the Emergency Department at the Ft. Wayne location, and the last device is for the Urgent Care facility in Marion. The MDSB10700 Scanner, Bladder, Biocon 700, with printer is only sold by one vendor on the Federal Supply Schedule 65IIA, which is Medline Industries, Inc. 5.BRAND NAME ONLY: This buy is estimated to exceed $25,000 and this documentation will be posted along with the RFQ on e-Buy, as required by FAR 8.405-6(b)(3). 6.Note that this determination satisfies the requirements of FAR 8.405-6. The requirement to prepare a written justification IAW FAR 6.303 does not apply to Federal Supply Schedules.
