- Experience and Qualifications Questionnaire Supplemental Specifications Plans Description of Work Schedule of Items to be returned with solicitation Wage Determination SF 1449 Cover Sheet/Form to be returned with quotation Terms and Conditions Representations and Certifications PROJECT TITLE: Furnish, Haul, and Spread Crushed Aggregate for Road Reconditioning of Dismal Creel Road, No. 201, at designated locations, Eastern Divide Ranger District, Giles and Bland Counties, George Washington & Jefferson National Forests. Work will require provision, placement, and minor associated site work related to placement of 5155 tons of crusher run and 165 tons of riprap over 7 miles of road. This solicitation, identified as AG-3395-S-13-0020, is issued as a Request for Quotation using simplified acquisition procedures. Issue date of this synopsis/solicitation is June 4, 2015. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are requested and a written solicitation will not be issued. RESPONSES WILL BE DUE BY 4:30 p.m., June 24, 2015. VENDOR MUST HAVE AN ACTIVE REGISTRATION IN THE SYSTEM FOR AWARD MANAGEMENT (http://www.sam.gov) in order to receive an executed Blanket Purchase Agreement from this solicitation. DESCRIPTION OF REQUIREMENT: Place 3" of crusher run aggregate on the traveled way, per "Typical Section" Detail (Sheet 4) and as directed by the COR. Place riprap at designated locations as described. Includes surface blading of the road, cleaning the ditches and culverts, and placement of 3" of aggregate at designated locations. LOCATION AND DESCRIPTION OF WORK Work will occur at the identified mile locations on Dismal Creek Road, No. 201, on the Eastern Divide Ranger District, Giles and Bland Counties, George Washington & Jefferson National Forests. DELIVERY: Contract time will be 30 calendar days from the receipt of an awarded purchase order by the successful awardee. Delivery times shall be scheduled so as to arrive at placement locations between the hours of 8 a.m. and 5 p.m. Monday through Friday unless prior arrangements have been agreed upon.Placement of aggregate shall be as directed by the Forest Service on-site representative. EQUIPMENT: Trucks used for hauling the aggregate material shall have a clean, tight bed, and shall have a minimum payload capacity of 15 tons. INSPECTION: Inspection of equipment shall be allowed during all segments of the delivery process. Acceptance of the in-place aggregate shall be contingent upon approval by the Forest Service. Materials and in-place product shall comply with VDOT specifications. MEASUREMENT: Certified weight tickets for the aggregate delivered and placed as directed will be the basis for payment. WHAT TO SUBMIT TO THE CONTRACTING OFFICE: • A completed and signed SF 1449 form, attached with this notice. • A completed Schedule of Items, attached with notice. • Representations and Certifications, attached with this notice. • Experience and Past Performance Questionnaire, attached with this notice. AWARD: Award will be made on basis of Best Value to the Government, price and other factors considered. Other factors include past performance and experience. Price is more important than non-price factors. The Government may choose to accept other than the lowest priced proposal when the perceived benefits of a higher priced proposal merit the additional cost. A fixed-price contract will be awarded from this solicitation.
