Description
1. This is a combined synopsis/solicitation for commercial supply prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This Request for Proposal (RFP) will also use the procedures under FAR Part 13.5 - Test Program for Commercial Items. In accordance with 13.106-2, the contracting officer may take into consideration information such as the contracting officer's knowledge of and previous experience with the services being acquired or other reasonable basis. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. The reference/solicitation number is VA268-15-Q-0043 and the solicitation is issued as a Request for Proposals (RFP). The combined synopsis/solicitation will result in a contract to the successful offeror. 3. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-82. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations (VAAR) Supplement can be accessed on the Internet at http://www.arnet.gov/far (FAR) and the (VAAR) (Federal Acquisition Circular) 4. This requirement is to establish a Requirements Contract for providing Centrifuges/Freezer equipment for Department of Veterans Affairs, Ralph H. Johnson VAMC, Research Department. 5. This requirement is a Total Service Disabled Veteran Owned Small Business Set-Aside (SDVOSB). Applicable NAICS Code is 334516 Size Standard is 500 employees. 6. Period of Performance: This requirement shall be for an anticipated period of performance from date of award through 90 days After Receipt of Order (ARO). It is anticipated that delivery of products will occur within 90 days ARO. 7. Place of Performance: Department of Veterans Affairs Ralph H. Johnson VAMC Research Department 109 Bee Street Charleston, SC 29401 8. The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation All questions and inquiries about this solicitation must be submitted in writing via email to elaine.dibucci@va.gov on or before June 11, 2015 by 10:00 AM. All responses to questions, which may affect offers, will be incorporated into a written amendment to the solicitation. Offerors must submit all proposals electronically by email to elaine.dibucci@va.gov referencing solicitation # VA268-15-Q-0043 in the subject line, on or before the proposal due date of June 19, 2015 at 4:00 PM. No hardcopy proposals will be accepted, and late offers will not be accepted. Offeror must submit a cost proposal as a separate document. This procurement is for brand name or equal products. Offeror shall provide a narrative to address each item and its brand name or equal compliance. Offerors must present clear evidence of ability to meet or exceed all requirements of this acquisition. Offeror must submit: 1. A completed cost sheet, located on Page 7. 2. A narrative addressing each item, presenting clear evidence of ability of vendor's product to meet the salient brand name or equal characteristics contained in this solicitation. 3. Past Performance Information: Proposal must list up to three contracts for products of a similar type, scope, size and complexity that are ongoing, or have been completed within the past five years. The following information shall be provided for each contract. oName and address of the contracting activity oName and telephone number of contact person oDates of contract performance (beginning and ending) oTotal dollar value of contract oBrief description of the project, scope and services offered Note: Please advise your references that they may be contacted from VA personnel regarding customer satisfaction with your products and services and your compliance with and fulfillment of the requirements of previous contracts. Information utilized in past performance evaluation will be obtained from the references provided by the Offeror, and may also be obtained from other sources such as customers known to the Government, others who may have useful and relevant information, and Government databases. 4. Socio-Economic Status - The Offeror shall provide the Socio-economic status of prime contractor 5. Offeror must clearly identify their DUNS# on their proposal. This acquisition will utilize the Lowest Priced Technically Acceptable (LPTA) procedure to make a best value award. Evaluation will be based on Technical, Past Performance and Cost Factors. A decision on the technical acceptability of each Offeror's quotations will be made. For those Offerors which are determined to be technically acceptable, award will be made to that vendor with the lowest overall price. While the Government will strive for maximum objectivity, the technical go/no go process, by its nature, is subjective; therefore, professional judgment is implicit throughout the evaluation process. The Government intends to make an award based on the initial quotations. If further questions are required, the contracting officer may contact the Offeror or Offerors. Award may be made without discussion. Award will be made to the responsible Offeror whose quotation conforms to all solicitation requirements, such as terms and conditions, representations and certifications, technical requirements, and also provides the best value to the Government based on the results of the evaluation 10. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. 11. The provision at 52.212-3, Offeror Representations and Certification, Commercial Items, applies to this solicitation. The contractor shall either: (1) return a completed copy of this provision with its proposal (a copy of the provision may be attained from http://www.arnet.gov/far); or (2) complete the Online Reps and Certs. Application at http://www.sam.gov and reference it in your proposal. 12. Statement of Need/Work Purchase of Mental Health Addition Laboratory Equipment Clinical Utility The goal of the new Mental Health Research Addition (MHRA) to the Charleston Ralph H. Johnson VA Medical Center is to bring together both clinical and basic science researchers to focus on cutting-edge mental health treatments. The function of the laboratories is to perform basic science research and to support clinical studies and formulate new ideas and concepts. With that in mind, the above equipment is needed to support those activities. With the equipment above purchased for use in the MHRA, both clinical and basic science researchers will be able to collaborate to collect and assess changes in biochemical pathways and clinical parameters with disease. With a focus on advancing an area of clinical investigation centered on pathology, application of the research program will address areas of pathology that cannot be addressed using current research applied in clinical practice and will provide substantial improvements on current practice. The clinical use will be on the development of new diagnostics and biomarkers and provide new insight into the pathophysiology of disease. "The purchase of the +4 °C refrigerators will be to provide the researchers with the capabilities to store chemicals, reagents and resources that are needed for the analysis of the tissues provided for the studies above. "The purchase of the table top centrifuges is needed primarily to spin red blood cells or bulky precipitates. High-speed units are used for most preparative applications and can collect microorganisms, cells, cellular debris, and precipitates. They can also separate viruses and cellular organelles, harvest yeast or bacteria from large cultures, obtain protein precipitates from pretreated samples, creating density gradients of subcellular material, and collecting viruses and bacteriophages from dilute solutions. The centrifuges will provide the investigators with the capability of spinning down blood and other samples from patients for analysis. The centrifuge must come with: -359150/ADAPT PURPLE 16MM 4PK -359157/ADAPT TAN 13MM 4PK -392806/MICROPLATE CARRIER ASSEMBLY "The purchase of the microfuges allows for the analysis of small samples. They are used in molecular biology and biochemistry to store and centrifuge small amounts of liquid. This is particularly useful when there is only a small amount of liquid in the sample or when small amounts of other liquids are being used, because microcentrifugation can be used to collect the drops together at the bottom of the tube after centrifugation. These will benefit the researchers by providing refrigerated microcentrifuges for the analysis of small samples and subcellular organelles. "The purchase of the CO2 incubators (a device used to grow and maintain microbiological cultures or cell cultures) allows for maintaining optimal temperature, humidity and other conditions such as the carbon dioxide (CO2) and oxygen content of the atmosphere inside. This will allow for the researchers to culture cell lines for analysis of pathways and mechanisms associate with the disease as well as culturing cells from patients for analysis. The incubators must include the following parameters: dual-stacked incubators with high temperature decontamination. 2 YR P&L warranty to be included. "The purchase of the spectrophotometer (nanodrop is a spectrometer which passes a beam of narrow-band (near-monochromatic) light through a droplet of water and measures the intensity of light transmitted through the sample) will allow for very accurate analysis of DNA samples that will be used for genetic analysis. DNA samples from patients can be isolated (under IRB protocols) for analysis of changes in the DNA related to cancer or other diseases, for identification of specific genotypes related to disease and determining potential treatment paradigms. The advantage of the nanodrop system is that by using only a drop of analyte solution, it conserves sample. The nanodrop must accommodate samples of 0.5 #I to 5 #I that can be pipetted directly onto the gold-colored sample plate for measurement, and then simply recovered using a pipette. 2 YR P&L warranty needs to be included. "The purchase of the ice-maker is to assist the researchers with protecting the samples for degrading or spoiling. An ice-maker is a critical need for a laboratory as it will help to preserve the samples taken from patients for study. Also needed is a 3YR P&L Warranty, 5YR on Compressor. 88lbs of ice per 24 hour period with up to 53lbs of built in storage. This request is to purchase the above equipment at this institution. These are standard laboratory equipment that is needed for the operation of the facility. Without which the research, collaborations and patient studies would not be possible. ?
