Description

285-15-RFP-0019 The Nashville Area Office of the Indian Health Service is proposing to locate a vendor to provide off-site pharmacy services for the Micmac Service Unit, 8 Northern Road, Presque Isle, ME 04769-2040. If you would like to submit a proposal on the work, please construct an offer for the compensation related to your services as defined in the following request for proposal and complete all of the documentation included in the package. The standard form (SF), SF-1449 must be completed along with any other signature blocks for consideration. No order under this Blanket Purchase Agreement shall exceed $180,000.00. Please respond no later than July 9, 2015 at 12:00 pm CST.
