Description
SSJ SF1449 Date: June 8, 2015 TO: Offerors FROM: Contracting Officer SUBJECT: Request for Quote The contractor shall provide the U.S. Department of Agriculture, Food Safety and Inspection Service (FSIS) with Maintenance Agreement Applied BioSystems (Life Technologies). A description of the requirement is included in the attached Statement of Work (SOW). EVALUATION: A. GENERAL Offerors proposals submitted shall be good for a period of 60 calendar days from the date specified in the SF 1449. The Offeror shall submit one (1) copy each of the technical proposal and price proposal. B. TECHNICAL PROPOSAL The equipment listed is mission critical and it is imperative that we receive a quick response by trained and certified Applied Biosystems (AB) only technicians. The vendor shall send a certified technician to perform service within 1 to 2 business days of maintenance service and provide services after normal regular business. The proposal shall clearly and fully demonstrate the offeror's capability and knowledge in regard to the requirements described in the RFQ. Please include the company: The (AB) Applied Biosystems Assurance Plan which includes 1(one) preventive maintenance (PM) service and the following: •Parts, labor and travel for remedial repair. •Priority telephone and email access to instrument technical support. •Telephone and email access to application technical support. •Smart Services remote monitoring, which provides for notification to customer of instrument failures or errors that are reported by AB's Smart Services monitoring software. The proposal shall include a purchased service contract package from Life Technologies to guaranteed priority response time of 1-2 business days. Failure to guarantee priority response time of 1-2 business days after receipt of a service call for instruments located at USDA, FSIS, OPHS, Eastern Laboratory, 950 College Station Road Athens, GA 30605 will result ineligible. C. PRICE PROPOSAL The offeror shall provide firm fixed unit prices for 5 years under the provision of a contract. Offeror shall complete the Pricing Schedule contained herein. Offeror must submit its price in accordance with the current market. The price shall include all costs such as labor, materials, preparation and shipment. D. RELEVANT EXPERIENCE AND PAST PERFORMANCE Offeror shall provide a list of at least three references for the same or similar work as described above. Offerors shall provide the customer's name and contact point, address, telephone number, contract number, contract value (award amount and current/final). Offeror shall also provide a list of any contract terminated and the reason for such action. INSTRUCTIONS: To aid in evaluation, RFQ shall be clearly and concisely written as well as being neat, indexed as appropriate and logically assembled. All pages of each part shall be appropriately numbered, identified with the name of the offeror, the date, and the solicitation number to the extent practicable. Quotes shall reference number 737213. Please note that this request does not commit USDA to pay any proposal preparation costs, nor does it obligate USDA to procure or contract for these services. DUE DATE: RFQ due no later than Noon Eastern Time, Friday, June 12, 2015, will be accepted either via fax at 301-504-4276 or by e-mail. No telephone response to this request for quote will be accepted. Quotes are to be submitted to the attention of the Point of contact(s) listed in this Notice. Offerors are responsible for confirming receipt of their quotes. To be awarded this contract, the prospective offeror must be registered in the System for Award Management (SAM). QUESTIONS: All questions are to be submitted either electronically to the attention of the point of contact listed in this Notice no later than Noon Eastern Time Thursday, June 11, 2015.
