Description
The Department of Veterans Affairs Medical Center (VAMC) at Salem, Virginia has a need for a Service Contract for Maintenance and Repair of Sterrad Sterilizers. The NAICS code for this requirement is 811219; size standard is $20.5m. This is not a request for bids, quotes, or proposals. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. The responses to this announcement will assist in determining the socio-economic set aside if feasible. VA is required by Public Law 109-461 to give first priority to SDVOSB and VOSB concerns if there should be sufficient firms available to compete under a set aside. If insufficient veteran owned and small business response is received to establish the basis for setting this action aside for other than large business participation then the action will be advertised as full and open competition, in which case, all interested parties responding shall be eligible to bid. - Essential technical capability: Must be licensed and/or certified to work with OEM proprietary software and make OEM maintenance and repairs. DRAFT STATEMENT OF WORK STERRAD STERILIZER MAINTENANCE VAMC SALEM, VA SCOPE OF WORK: Contractor shall provide all scheduled preventive maintenance, repairs, repair parts, calibrations, and phone support as need to maintain full operational capacity of three (3) Sterrad sterilizers, model NX. Contractor shall have full access to the original manufacturer's software for calibration and repairs as required; no subcontracting of any portion of this service will be authorized by the Government. PERIOD OF PERFORMANCE: Base Year:October 1, 2015 through September 30, 2016 Option Year 1: October 1, 2016 through September 30, 2017 Option Year 2:October 1, 2017 through September 30, 2018 Option Year 3:October 1, 2018 through September 30, 2019 Option Year 4:October 1, 2019 through September 30, 2020 GENERAL REQUIREMENTS: In accordance with the manufacturer's requirements, NFPA, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and local policy, all scheduled and unscheduled maintenance is required on this equipment, with appropriate documentation. Additionally, all service (including parts) shall be equal to or greater than the original manufacturer's specifications. Contractor shall ensure their personnel meet the privacy standards as set forth by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) with respect personal and confidential information which may be accessed during medical equipment maintenance and/or repair. The Contracting Officer/ Contracting Officer's Representative (CO/COR) will provide additional guidance regarding training in this aspect; the COR will be appointed by the CO upon award of the contract. SPECIFIC REQUIREMENTS: Contractor shall provide the Biomedical Engineering Service COR with individual written reports which describe the maintenance and repair service performed on the equipment in sufficient detail so as to be acceptable by field inspectors of JCAHO. This shall include a list of all parts replaced and all service performed, as well as a statement that the equipment is operating per manufacturer's specifications after repair. The service report will be signed by the contractor's service technician and by a designated VAMC Salem Biomedical Engineer. A copy will be given to the COR for their internal records. All technicians providing service on this equipment shall be factory trained by the original manufacturer of the equipment. Documentation of this training shall be available upon request. Except by special alternative arrangement, scheduled maintenance shall be provided only during Contractor's normal working hours (8:00 a.m. to 4:30 p.m.), Monday thru Friday, (excluding holidays) local site time. All work shall be performed at the VA Medical Center, 1970 Roanoke Blvd., Salem, VA 24153. Prior to award, Contractor shall provide evidence that a facility, to include personnel, test equipment, parts inventory, training certificates, licenses, and technical documentation, is available for inspection by VA Medical Center personnel, as well as the ability to show written evidence to technical qualifications of personnel, test equipment (and calibration documentation) licenses and availability of parts. Parts availability shall have written documentation from either the equipment manufacturer or authorized parts supplier. In either case, Contractor shall have the availability of parts within 24 hours after initial call. For contracts for maintenance and repair services from other than the original manufacturer or a designated representative, Contractor shall be fully responsible for obtaining all technical documentation necessary to fulfill contractual obligations. Contractor shall warrant that services performed under this contract shall be performed in a good workmanship manner and shall conform to the standards of the industry. When services and parts are required as a result of accident, abuse, misuse, or negligence by other than the Contractor or his representative, and are not included as a part of PM inspection or service calls, such needs shall be reported to the CO/COR along with estimates of charges. The CO/COR will evaluate these needs and authorize appropriate action. The parts shall be installed under the terms of the contact and each invoice shall include a legible itemized list of labor charges and parts costs. The Government will incur no charges for service and parts due to accident, abuse, misuse, or negligence by the Contractor or his representative. Contractor shall guarantee that all equipment covered in this contract shall meet or exceed manufacturer's performance specifications at the contract expiration date. At the end of the contract term and within the two weeks following the award to the new Contractor, there shall be a final inspection of all equipment at which the Contractor, the new Contractor, and the COR shall be present. Any deficiencies noted shall be the responsibility of the previous Contractor and shall be corrected by the terms of this contract.
