- NM Army National Guard Occupational Health Services has a requirement for Medical Surveillance Exams for full-time support personnel, (not to exceed 160 Personnel with a period of performance to begin no later than 27 July 2015 through 4 August 2015). The acquisition is reserved for small business concerns; in the event of insufficient participation from small business concerns, a participating large business concern may be considered. The contractor shall provide all personnel, supervision, and any items and services necessary to perform the services. The solicitation will be made available, on the Army Single Face Industry (ASFI) no later than 10 June 2015, with a closing date of 23 June 2015 at 4:30 pm (MST). All prospective offerors interested in submitting a proposal on this solicitation must be registered on The System Award Management database. The website to register is: www.sam.gov in accordance with DFARS 252.204-7004. Lack of registration will make an offeror ineligible for award. All offerors are responsible for downloading the solicitation from this website and to frequently monitor that site for any amendments to the solicitation. Failure to respond to electronically posted solicitation and associated amendments prior to the date and time set for receipts of proposals may render your proposal non-responsive and result in the rejection of the same. The offeror must also be registered on Wide Area Work Flow (WAWF), the Army's electronic invoicing system prior to award. The website to register is: www.wawf.eb.mil.
