- RFQ-MA-15-00008 Attachment 2 Statement of Work Attachment 1 Clauses Combined Synopsis/Solicitation This solicitation is issued as a request for quotation (RFQ). The EPA intends to award a Firm Fixed Priced Order for a Mobile Biology Laboratory. This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this announcement. This announcement constitutes the only solicitation. This solicitation document and incorporated provisions and clauses (Attachment 1) are in effect through Federal Acquisition Circular 2005-82. You must be registered at SAM.gov, http://sam.gov, as a small business with NAICS Code 336211 to be considered for award of any order resulting from this solicitation. Description of item(s) to be acquired: The United States Environmental Protection Agency (EPA) requires a Mobile Laboratory Vehicle to serve as a flexible platform for a variety of studies. The Contractor shall customize and deliver a Mobile Laboratory as required in Attachment 2, Statement of Work. Delivery is required within 120 days of receipt of the order. Deliver to: EPA Region1 Laboratory 11 Technology Drive North Chelmsford, MA 01863 EPA will accept delivery within 30 days upon condition that the Mobile Laboratory Vehicle meets all requirements. Proposal Preparation Instructions: a. Please provide a firm fixed price quote for the services identified in Line Item 0001 of the Attached Form 1449. The price shall include all costs associated with the required effort to include all labor, materials, travel and delivery costs. The offeror may use their standard commercial format for submittal of their price quote, however, your quote must be broken out by material, labor, travel, delivery and any other associated costs. b. Please include a description of your approach to meeting the requirements. Limit this description to 10 pages. Include in your description how you will meet each of the following requirements as defined in the Statement of Work, Attachment 2: 1.Chassis Requirements 2.Laboratory Body 3.Other Exterior Body Requirements 4.Heating/Cooling and Ventilation 5.Distribution of Interior Space 6.General Requirements for Interior Furnishings 7.Plumbing Requirements 8.Electrical Power Requirements 9.Generator Compartment c. Please include examples of previous work providing a similar Mobile Laboratory. Include at least one example but not more than three. d. Please include at least one reference for a similar effort, but no more than three references. Include a name, phone number and e-mail address for each reference. e. Please include your DUNS number with your quote. f. Offerors shall comply with instructions identified in provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, for completion of Representations and Certifications. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov, or http://sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (p) of this provision. g. Offerors are also required to complete the provision at EPA-K-04-101, Representation by Corporations Regarding a Felony Conviction under Federal Law or Unpaid Federal Tax Liability., which can be found on page 27 of the Provisions and Clauses. Please check off "is" or "is not" for questions (b) (1) and (2). Award Criteria: In accordance with FAR 13.106-2 an award will be made to the offeror who can provide the best value. When determining best value the following evaluation factors will be considered along with price: past performance and compliance with all requirements as stated in the RFQ. Evaluation factors other than price when combined are approximately equal to price. Offerors who do not provide all of the requested information will not be considered for award. The Contracting Officer may contact offerors to request clarification of information provided to better understand your quote or may request information that appears to be inadvertently omitted. Only one award will be made under this solicitation. Award will be conducted under the provisions of FAR Part 12, Commercial Items, and FAR 13, Simplified Acquisition Procedures. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Notice to Offerors: The Government intends to award a Firm Fixed Price contract for this requirement. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
