- THIS IS A MODIFICATION TO THE ORIGINAL SYNOPSIS TO REFLECT THE CHANGES IN CONTRACT TYPE, ESTIMATED NUMBER OF PATIENTS, ANTICIPATED ISSUE/CLOSING DATES, AND NAICS SIZE STANDARD The Department of Veteran Affairs, Service Area Office Central, on behalf of Veterans Integrated Service Network (VISN) 12, anticipates soliciting proposals and consequently awarding a contract for Home Oxygen Services and Home Medical Equipment (HME) Services for beneficiaries of the medical centers and outpatient clinics covering three (3) states. The medical centers locations that comprise VISN 12 include; Milwaukee, Madison, and Tomah, WI, North Chicago, Hines and Jesse Brown, VAMC's IL, and Iron Mountain, MI.. These comprehensive services will provide Home Oxygen/HME Services to the estimated 3,500 oxygen using patients. The solicitation will be conducted in accordance with FAR Part 12 (Acquisition of Commercial Items) and will be evaluated with procedures therein. The solicitation will be advertised as Full and Open Competition. Offeror shall provide all labor, management, health care professionals, equipment, supplies, and transportation to fulfill the level of care and range of services necessary to furnish home oxygen and HME services to the veteran beneficiaries of VISN 12. The Government intends to award firm-fixed price Requirements contract(s) in accordance with all terms, conditions, provisions and specifications of the solicitation. The term of the contract is for one year with four (4) one year options. NAICS code is 621610 - Home Health Care Services with size standard of $15 million. The solicitation documents will be issued on or about June 10. 2015. Proposals will be due on or about July 10, 2015. This notice is for informational purposes only and is not a request for proposals. The Government is not obligated to issue a solicitation as a result of this notice. Solicitation documents may be obtained electronically at Federal Business Opportunities website at www.fbo.gov. Prospective contractors must be registered in the Central Contractors Registration (CCR) database at www.ccr.gov in order to obtain solicitation documents. CCR and Vetbiz registrations must be current and active to be eligible for contract award.
