W-9 required for consideration Required Vendor Forms Milk/Dairy Delivery Scope of Work THIS IS A PROCUREMENT OPPORTUNITY WITH ADAMS AND ASSOCIATES AND THE GADSDEN JOB CORPS CENTER AND NOT WITH THE U. S. FEDERAL GOVERNMENT. We are currently soliciting bids for a Blanket Purchase Order (BPA) for the Gadsden Job Corps Center for the month of July 2015 with additional four month and 6 month options. Please see below the scope of work and service: Scope of Work: 1. Provide 1% 5 gallon bag milk, 2% 5 gallon bag milk, 1% chocolate bag milk and individual pint size 2% milk to the center's cafeteria on a weekly basis. 2. Please list locked in pricing for each of the milk selections for the entire period. Please include with your bid: W-9; Vendor Questionnaire along with bid (attached) If you are a HUB Zone or disadvantaged (8a) business; please include a copy of your certification. For a tour of the scope of work involved, please feel free to call and make an appointment. All quotes must be faxed, mailed or e-mailed by: Friday, June 19, 2015, at 5:00 PM CST to meet deadline.
