RFQ # DTPH5615Q00012 Please read this document in its entirety as it contains pertinent information (attachment, evaluation procedures, due dates, etc.) regarding this Request for Quotation (RFQ) # DTPH5615Q00012. TITLE: Analysis of a Ruptured DOT 3AL Cylinder PURPOSE OF SOLICITATION: The U.S. Department of Transportation (DOT), Pipeline and Hazardous Materials Safety Administration (PHMSA), Office of Hazardous Materials (PHH) has a requirement for the analysis of a ruptured DOT 3AL Cylinder. You are invited to submit a quotation in response to PHMSA Request for Quotation (RFQ) # DTPH5615Q00012. Engineering Services NAICS Code 541330 (SIZE STANDARD = $15 Million). Please read the attached (ATTACHMENT# 1) Statement of Work (SOW). ------------------------ CONTRACT TYPE AND AWARD: PHMSA intends to issue a firm fixed-price purchase order as a result of this RFQ. ------------------------ EVALUATION PROCEDURES: Award will be made to responsible contractor who presents the best value quote. PHMSA will use tradeoff analysis when making the best value determination and award based on the following factors to evaluate offers: FACTOR 1: TECHNICAL CAPABILITY: (a) Contractor to provide facilities to assure prompt and accurate completion of each task identified in the attached SOW. (b) The contractor must have a demonstrated background in metallurgical failure analysis and root cause determinations relating to the failure of metallic equipment, specifically as stated in attached SOW and determination of failures of metallic equipment, specifically: • Failure of metallic pressure vessels • Failure of metallic equipment in compressed gas service • Failure of metallic pressure vessels in compressed oxygen service • Failure analysis of aluminum alloy compressed cylinders in oxygen service • Familiarity with DOT 3AL cylinder specifications FACTOR 2: PAST PERFORMANCE: Quoters must submit evidence of successful past performance on contracts for work similar to the tasks described in this announcement's statement of work. Each item should include, at a minimum, the contract number, a brief description of the work performed, total value of the contract, and the client company or government agency's name. Quoters should also provide at least one telephone reference for each past performance item. FACTOR 3: PRICE: Competitive pricing and discounts are highly encouraged. ORDER OF IMPORTANCE: Evaluation Criteria 1 and 2 are of equal importance. When combined, Factors 1 and 2 are significantly more important than Factor 3 (Price). ------------------------ QUOTATION CONTENTS: 1. Quotations must reference/include the following: a. PHMSA RFQ # DTPH5615Q00012 b. Contractor DUNS Number c. Contractor Tax ID Number d. Quotation Date e. Quotation Expiration Date f. Delivery: The contractor is required to complete the work within 60 calendar days from the date the contract is issued. ------------------------ DELIVERY: The contractor is required to complete the work within 60 calendar days from the date the contract is issued. The final report must be sent as a draft to the COR, for review and approval. Once the COR has approved the draft, the contractor must submit two hard copies and one CD as a final report. This final report must be sent to the COR within 10 working days of the completion of all the tasks. In addition to this, the contractor must send one electronic copy to the Contract Administrator, Shaukat Mirza at Shaukat.Mirza@DOT.gov and a copy to the Contracting Officer, Mr. Warren Osterberg at Warren.Osterberg@DOT.gov. ------------------------ DEADLINE OF QUOTATIONS: Interested contractors must submit one electronic copy of their quote via email to Mr. Shaukat Mirza Shaukat.Mirza@dot.gov no later than 6:00pm Eastern Time on Monday, June 22, 2015.
