- Purpose of this Synopsis / Notice of Intent (NOI) This Synopsis / NOI is being issued by the Defense Health Agency (DHA), Contract Operation Division Falls Church (COD-FC) to conduct Market Research on Industry’s Capabilities in providing Telementoring Services. Telementoring Services include all the technical services and supplies needed to develop the capacity to safely and effectively treat chronic, common, and complex diseases using a Telementoring approach. The DHA COD-FC intends to issue a 12 Month Base period plus four 12 Month Option Periods Sole Source Contract under FAR 6.302-1 “Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements” to the University of New Mexico Health Sciences Center, 1001 Medical Arts Ave. NE - Bldg. 424 Albuquerque, New Mexico 87102. Scope The Clinical Support Division’s Chief of Condition-Based Specialty Care will use the approach to work on one of the four strategic focus areas of the Military Health System (MHS): Improving quality outcomes for condition-based care. The Clinical Support Division currently is working with various stakeholders on a pain-management Telementoring effort to support this focus area. The contractor support team will complete the pain-management work and then focus on other strategic focus areas. Depending on the specialty care mentoring that is needed; this action will provide DHA the ability to bring in the desired expertise. The contractor shall provide subject matter and expert knowledge in Telementoring and education practices, procedures and protocols related to the implementation of proposed Telementoring Project initiatives. The contractor shall provide employee staff, including a physician program director, physician project manager, physician trainer, allied health professional, physician specialist, administrator, and tech support to support the tele-mentoring initiatives. The contractor shall perform a technology needs assessment to deliver a technology model for specialist hubs and remote primary care provider spokes (one-to-many) to pilot a tele-mentoring initiative. The contractor shall train doctors, nurses, physician assistants and other medical personnel in rural and underserved areas to work together to manage patients, thereby expanding the MHS capacity to deliver high-quality chronic care to those most in need. The contractor shall train primary care champions to assist in moving specialty expertise to the front lines, including nurses and other health workers. Telementoring is part of an initiative to 1) improve primary care knowledge, self-efficacy, and attitudes; 2) leverage technology to facilitate case-based training by specialists to primary care providers in their local setting; 3) decrease staff turnover; 4) increase provider satisfaction; 5) decrease the number of non-essential referrals to specialty care in the direct and purchased care system; and 6) decrease the cost of travel for clinicians to obtain continuing medical education. If implemented enterprise-wide, this initiative will improve quality outcomes across the MHS. NAICS Code for this action is 611310 -- Colleges, Universities, and Professional Schools. How to Respond This is a Notice of Intent ONLY. It is not a request for competitive proposals. It is issued solely for Information and Planning Purposes – It does not constitute a Solicitation nor does it restrict the Government as to the ultimate Acquisition Approach. In accordance with the Federal Acquisition Regulation (FAR) 15.201, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Should your firm desire to submit a capability statement, it should demonstrate the Offeror’s experience and expertise to provide the services identified above within the time frame required and without substantial duplication of cost to the Government. The statement shall include business size, ability to self-perform work; layers of subcontracting; and any other information indicating a legitimate ability to meet these specific requirements. Statements should not exceed 5-10 pages in length. Responders are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this Synopsis / NOI. All costs associated with responding to this Synopsis / NOI does not preclude participation in any future Request for Proposal (RFP), if any is issued. The formal closing date for this Synopsis / NOI and submission of Responses is Thursday, 25 June, 2015 at 2:00PM EST.
