Performance Work Statement Please also submit this itemized cost sheet based on the Performance Work Statement and the site visit. RFQ for Hoods and Ducts COMBINED SYNOPSIS - SOLICITATION FOR COMMERCIAL ITEMS (IAW FAR 12.603 (c)) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is FA441816HOODS and is being issued as a Request for Proposal (RFP). The RFP format is attached to this combination solicitation synopsis that will used to submit proposals. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82, dated 7 May 2015. (iv) This acquisition is 100% set-aside for small business. The associated NAICS code is 561790 with an $7.5 million size standard. (v) Contractors shall submit a proposal using the provided price sheet for the description of requirements to include all tools, supplies, equipment and labor necessary to perform in accordance with the attached Performance Work Statement. A site visit will be conducted on June 17, 2015. All contractors shall meet on the second floor of bldg 503, Charleston AFB. (Note: The site visit is not mandatory but highly recommended) Contact, SrA Matenoa Taufa, at 843-963-5166 or matenoa.taufa@us.af.mil NLT 1200 on June 15, 2015 with the names of all who plan to attend. You must have a valid driver's license, proof of insurance, and vehicle registration to get on base. All responsible sources may submit a proposal, which shall be considered. (vi) FA441816HOODS (Cleaning of hoods and ducts) on Joint Base Charleston Air Base (AB) and Weapons Station (WS), South Carolina. (See attached performance work statement). (vii) The period of performance is one (1) Base performance period of 12 months plus (4) Option performance periods of 12 months each. (1 Oct 15 - 30 Sep 16; 1st option is 1 Oct 16 - 30 Sep 17; 2nd option is 1 Oct 17 - 30 Sep 18; 3rd option is 1 Oct 18 - 30 Sep 19; 4th option is 1 Oct 19 - 30 Sep 20). (viii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1, Instructions to Offerors - Commercial. (ix) FAR 52.212-2, Evaluation - Commercial Items Evaluation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following evaluation criteria shall be used to evaluate offers in the following relative order of importance: 1. Technical - contractor must be trained and certified in accordance with the National Fire Protection (NFPA) Code 96, Chapter 11 and South Carolina State Fire Marchal's office. Contractor must submit certification with proposal. 2. Past performance; and 3. Price - The Government will evaluate the total price of the offer for award purposes. Technical capability (pass or fail), Past Performance is significantly more important than Price. If the lowest priced evaluated responsible offer is judged to have a Substantial Confidence Performance Assessment Rating and meets the technical capability, then that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made without further consideration of any other offers. Offeors shall provide no more than 3 RECENT AND RELEVANT REFERENCES for Hoods and Duct Preventative Maintenance. "Recent" is defined as within the past three years. "Relevant" is defined as performance involving the same or essentially the same magnitude of effort and complexity as the PWS requires. Options: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (x) All offerors shall include a completed copy of FAR 52.212-3, Offeror Representation and Certifications -- Commercial Item or complete electronic annual representations and certifications at the System for Award Management http://www.sam.gov (xi) The clause at FAR 52.212-4, Contract Terms and Condition-Commercial Items, applies to this acquisition. (a) The Government may extend the term of this contract by written notice to the Contractor within 15 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (End of clause) • AFFARS 5352.223-9001, Health and Safety on Government Installations (a) In performing work under this contract on a Government installation, the contractor shall: (1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract. • AFFARS 5352.242-9000, Contractor Access to Air Force Installations (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. • FTE Reporting - 1. (a) The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the [NAMED COMPONENT] via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil. (b) Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk." ---------------------------- *Reporting Period: Contractors are required to input data by 31 October of each year. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
