Description

STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Building #28 Interior Renovation Project at VAMC, Northport, 79 Middleville Road, Northport, NY, 11768. 2. Scope of Work: The Government intends to award a Firm Fixed-Price Contract to provide all labor, materials, and equipment necessary to fully renovate the interior of Building #28. The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW). The following work will be performed in order to accomplish the renovation. o Prepare site and cordon off working area in accordance with established safety regulations. o Elimination of safety hazards. o Dust Mitigation Measures to be conducted during all phases of work. o Terminal Clean-up at the completion of each work period. o Demolition of existing bathroom and fixtures. o Selective demolition of interior walls and ceiling. o Removal of 350 square feet of existing flooring. o Removal of existing electrical panel. o Removal of five existing light fixtures. o Remove seven exterior windows. o Remove one exterior door o Remove existing interior, non-load bearing walls. o Selective demolition of exterior portico. o Frame interior walls according to design plan. o Install new doors one exterior, three interior according to schedule. o Install new bathroom sink, urinal and toilet according to design plan. o Install six new 180 mile per hour impact rated replacement windows. o Seal wall cavity of approximately 12 sq.ft. with brick and mortar. o Frame and set new portico. o Relocate electrical panel. o Install wiring from junction box to main panel and make connections to breakers. o Installation of eight new light fixtures in ceiling. o Install 47 square feet of new concrete section to level floor under portico. o Install 375 square feet of new commercial grade slip coefficiency rated ceramic flooring. 3. Performance Period: The contractor shall commence work within thirty (30) days following the contract award. Work shall complete within sixty (60) days of commencement unless delayed by unforeseen circumstances. 4. Type of Contract: Firm-Fixed-Price. 5. Evaluation Criteria: Lowest Price-Technically Acceptable. 6. Place of Performance: VA Medical Center, 79 Middleville Road, Northport, NY 11768 It should be noted that this contract does not involve access to VA sensitive information and does not involve access and/or connection to the VA information system.
