This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation for commercial items is in accordance with Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition Procedures. This is a Request for Proposal (RFP) for furnishing commercial items. The National Cemetery Administration is soliciting offers under solicitation number VA786-15-R-0256 for the following cemetery: Marion National Cemetery 1700 East 38th Street Marion, IN 46952 PH: (765) 674-0284 Period of Performance; 45 calendar days after notice to proceed This acquisition is 100% set aside for Service Disabled Veteran Owned Small Businesses. The Contractor(s) gaining award shall provide all labor, tools, materials, and equipment necessary to accomplish tree and/or stump removal, pruning, trimming, cabling, cavity repair, root pruning, and the removal and disposal of resulting waste and debris for the National Cemeteries listed below, on a one-time basis. The contractor shall provide all professional advice/guidance, parts, materials, equipment, and personnel, to provide these services. A.2 THE NCA MISSION: The National Cemetery Administration honors veterans with a final resting place and lasting memorials that commemorate their service to our Nation. National cemeteries are national shrines. The standards of maintenance, appearance and operational procedures performed by the Contractor at these cemeteries shall reflect this nation's concern for those interred there. For this reason, the Contractor's strict adherence to the specifications shall be required and shall be essential. A.3 EVALUATION OF OFFERS Method of Evaluation - Cascade Procedures This a competitive Request for Proposals (RFP) using a Three (3)-Tier cascading set-aside procedure. The Government will make award at the highest possible Tier. The set-aside requirement Tiers will be as follows: 1st Tier: Service Disabled Veteran Owned Small Business (SDVOSB) 2nd Tier: Veteran Owned Small Business (VOSB) 3rd Tier: All small businesses Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) and Veteran-Owed Small Businesses (VOSBs) must be verified for ownership and control and listed in the Vendor Information Pages at https://www.vip.vetbiz.gov prior to the due date for proposals. Award will be made to a responsible offeror on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-price factors; only where adequate/sufficient competition is received at a given Tier. Adequate competition exists if; (1) proposals are received from at least two qualified / technically acceptable responsible offerors; (2) each of whom can satisfy the contract's requirements independently; (3) both contend for a contract which will be won by the offeror who submits the lowest evaluated price meeting or exceeding technical requirement and; (4) price is reasonable as determined in accordance with FAR 19.202-6. If the above is satisfied in Tier 1, Tier 2 and Tier 3 proposals will not be considered. If not, Tier 2 will be added to the evaluation process. If no acceptable offers are in Tier 2, Tier 3 will be added to the evaluation process. Evaluation Process for Award The Contracting Officer will evaluate proposals to assess offerors' ability to perform the contract successfully. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the requirements of the Statement of Work (SOW) sections of the solicitation, which will be most advantageous to the Government, price and other factors considered. Award will be made to the Lowest Price Technically Acceptable (LPTA) Offeror. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-price factors. For evaluation purposes the lowest price will be determined by calculating the total price of Contract Line Item Numbers (CLINs) in order to determine a total amount. The following factors shall be used to evaluate offers on a lowest cost, technically acceptable bases in accordance with FAR 15.101-2: A.Price i.Price analysis will be conducted to determine a fair and reasonable price. The Government may use various price analysis techniques and procedures to ensure a fair and reasonable price. B.Technical Qualifications: Technical capability will be evaluated to determine the extent to which it demonstrates a clear understanding of all features involved in performance of the requirements identified in the SOW. The proposal should not simply restate the Government's requirements, but it should describe, in detail, how the Offeror intends to meet the requirements. Offerors shall provide information for the following sub-factors, which are weighted equally: i.Experience of company and/or subcontractors in performing this type of work: have you or your proposed subcontractor done/performed projects of equal to or of greater complexity as the requirements of the Scope of Work? Please explain. Contractor shall submit a list of compaction grouting projects undertaken in the past three to five years along with their proposal. At least three of the grouting projects specified should be of equivalent difficulty and/or scope. The list shall include a description of the project, relative size, and contact person with phone number. Contractor will not be considered technically acceptable without a minimum of three years of experience in compaction grouting. ii.Performance Plan: The contractor shall provide in writing a detailed work plan which presents the contractor's plan for completing the work. The contractor's plan shall be responsive with the scope of work and describe, in detail, the approach to be used for each task required. For each task, the contractor shall identify all necessary subtasks (if any), associated costs by task, and milestone dates. Identify any subcontractor(s) to be used in performance of this contract, along with the subcontractor's socio-economic status (i.e. Disabled/Veteran owned, Woman owned, Small, Large Business, etc.). If using a subcontractor, list the type and percent of work you will perform and that they will perform. C.Past Performance: Past performance will be evaluated for quality, timeliness (e.g., recent), problem resolution, business relations, customer service and relevance (i.e. experience in providing services similar in size, scope, and complexity as described in the SOW). If no past performance information is readily available, the Offeror's past performance will be evaluated as Neutral. The Government will research information and data bases to aid in establishing contractor's responsibility and ability to perform. The databases include, but are not limited to Experian, D&B, PPIRS, EPLS, VetBiz and SBA. Bonding: The Government retains the right to require bonding, (bid, performance, payment) if it is determined to be in the interest of the Government. The Government will reimburse 100% of the bonding price. Failure to receive or qualify for bonding will result in removal from further consideration for award. A.4 SUBMISSION OF PROPOSALS 1.All proposals must be submitted through the Department of Veterans Affairs Electronic Management System (eCMS) Vendor Portal website to be considered. 2.Please go to the VA eCMS Vendor Portal website at https://www.vendorportal.ecms.va.gov to register. Once on the webpage, proceed to the Vendor Portal Login section located on the far left side of the webpage and click on 'Request a user account' to register. 3.In the event an Offeror is unable to submit a proposal through the Vendor Portal, prior to the proposal closing date and time, contact the VAAS helpdesk at 1-877-634-3739, or via email at VA.Acquisition.Systems@va.gov. Submission of proposals by email will not be accepted. Proposal transmission/uploads must be completed by the date/time specified. Late or incomplete Proposals will not be considered. 4.If Offerors are still unable to submit a proposal through VA eCMS Vendor Portal, the Contracting Officer may elect to accept submission of a CD version of the proposal package as long as VA eCMS Vendor Portal registration requirements have been fulfilled and Offerors have contacted the VAAS helpdesk for assistance in their submission of a proposal. A copy of the email correspondence with the VAAS helpdesk will be forwarded to the Contracting Officer, John M. Carlock, at john.carlock@va.gov as proof of email correspondence. If a phone conversation with the VAAS helpdesk proves unsuccessful in an Offeror's ability to submit a proposal prior to the closing date via Vendor Portal, the Offeror shall submit to the Contracting Officer correspondence that contains the date, time, and name of helpdesk representative the Offeror contacted to include the reasons why the Offeror could not submit a proposal via the Portal. This document will be submitted together with the CD version of the proposal. In the event an Offeror has not requested proposal submission assistance to the VAAS help desk prior to the closing date, nor has submitted correspondence that identifies reasons why the Offeror could not submit a proposal via Vendor Portal, the Offeror will be considered non-responsive and the CD version of the proposal will not be accepted. 5.If a CD version of the proposal will be accepted by the Contracting Officer, the electronic version of the proposal may be submitted as an MS Word document in.doc or.docx format or Portable Document Format (PDF). If PDF is preferred, then each Section submitted shall be a separate file. For example, all required licenses, technical proposal, Past Performance Questionnaires, shall all be under separate files and shall be classified as such. The files shall be categorized in a manner where it is easy to ascertain. Offerors who meet stated requirements may mail or hand carry the CD version of their proposal to the address specified in Block 7 of SF 1442. Offerors are responsible to ensure proposals are received no later than the date and time specified in the solicitation. Late or incomplete Proposals will not be considered. See FAR 15.208 - Submission, Modification, Revision, and Withdrawal of Proposals. All offers are subject to all terms and conditions of this solicitation. The proposal package shall contain the following: a.Completed and signed SF 1449 with all required blocks completed on Page 2, to include Total Price in block 17 and printed name, authorized signature and date in blocks 30a, 30b and 30c. b.Technical Qualifications in a written narrative as specified on pages 9-10 for the Government's evaluation. c.Three (3) past performance references within the last 3 years that are similar in size and scope to this solicitation. Use the Past Performance Questionnaire (Attachment A). Prepare one for each reference. d.Completed Schedule of Prices in.doc or.docx format or PDF. e.Required representations and certifications f.Acknowledgement of any amendments Failure to submit all required documentation may result in your submission being determined technically unacceptable and removed from further consideration. PROPOSAL FORMAT 1.Page size shall be no greater than 8 1/2" x 11". The top, bottom, left, and right margins shall be a minimum of one (1) inch each. 2.Font size shall be no smaller than 11-point. Arial or Times New Roman fonts are required. Characters shall be set at no less than normal spacing and 100% scale. 3.Tables and illustrations may use a reduced font size no smaller than eight (8)-point and may be landscape. 4.Line spacing shall be set at no less than single space. 5.Each paragraph shall be separated by at least one blank line. 6.Page numbers, company logos, and headers and footers may be within the page margins only and are not bound by the 11-point font requirement. 7.Footnotes to text shall not be added. 8.If the Offeror submits annexes, documentation, attachments or the like, not specifically required by this solicitation, such will count against the offeror's page limitations unless otherwise indicated in the specific volume instructions below. 9.Pages in violation of these instructions, either by exceeding the margin, font, or spacing restrictions or by exceeding the total page limit for a particular volume, will not be evaluated. 10.The following page limitations are applicable to this procurement: Volume FactorPage Limitations Volume ITechnical20 pages single-sided Volume IIPast Performance6 pages single-sided (Excludes Past Performance Questionnaires) Volume IIIPrice Schedule, SF 1442, Amendments, Required Certs and Reps, Licenses, Bonding, Insurance, etc ¦No Limit A cover page and/or table of contents which are not required, will be included in the page count of the Technical Volume if included in the Volume. A glossary of abbreviations or acronyms will not be included in the page count of the Technical Volume. Proposal packages that do not contain all the above materials may be rejected. Proposal packages that fail to include sufficient Technical, Past Performance and/or Price information for a competitive evaluation may result in the proposal being rejected and not considered. Proposals that are not received completely by the time and date specified in Block 13 of the SF 1449 will be rejected and shall not be considered. The following list is not intended to be all inclusive. It is intended to give offerors some idea of what should be provided in each volume for evaluation. Technical Volume: Offerors shall provide the following: The project plan for managing this project, with any proposed subcontractors listed and their socioeconomic status (i.e. Disabled/Veteran owned, Woman owned, Small, Large Business, etc) - list the type and percentage of work the Prime Contractor will perform and that the subcontractor will perform. Also provide information on your company's capabilities; e.g., years in business; type and age of equipment/vehicles to be used on the project; all licenses, permits, insurance information of the offeror and personnel; key personnel information; training and experiences of personnel who will be working this project; company's experiences in working similar/like type projects; number of jobs you have completed that are similar to this request; your quality assurance plan; any phasing plan, number of projects currently ongoing; any affect they may have on this proposal; any surveyor/survey company you will be using, your projected duration for this project. Submit a proposed project work schedule with timeline. Please provide any additional information that will assist in understanding your plan/approach and superiority in providing services. Past Performance Volume: Offerors shall provide at least three (3) references/referrals that specifically address offeror's performance on projects within the last three (3) years that are similar or of the same type in terms of scope, size, and pricing (e.g., Scope - all types of work this project is asking for as described in the solicitation; Size - approximately the same unit quantities as this project is asking for; Price - approximately the same price or better than offeror's price proposal for this project). Offerors are encouraged to submit referrals that specifically address experiences with cemeteries. Contact information provided with referrals must be reachable by telephone or email. The Government will not research contact numbers or emails. If a referral cannot be reached with the information provided, offeror will not get credit for that referral. Offerors shall also provide the number of projects they are currently working on for NCA, with the corresponding Contracting Officer's name, telephone and contract information. Offerors shall provide a list of the subcontractors they commonly use on their projects. Offerors shall also provide contact information on any active or pending assignment of claims or factoring agreements. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror will not be evaluated favorably nor unfavorably on past performance. Price Volume: Offerors shall provide a price proposal based on the price schedule. The lowest priced proposal will not necessarily get this award. This is not an Invitation For Bid (IFB). ? SCOPE OF WORK Contractor shall provide all labor, supervision, equipment, materials and supplies for tree work at: Marion National Cemetery 1700 East 38th Street Marion, IN 46952 All work shall be conducted under the direction of an International Society of Arboriculture (ISA) certified arborist, and includes: 14 trees to be removed, with their stumps (plus one additional stump); 119 trees to be crown-clean pruned; 4 trees to be structurally pruned; 8 trees to be cabled and/or braced, as needed; 3 trees with wounds to be scribed. Contractors are highly encouraged to visit the site so they can assess the individual trees as they prepare their bid. Please contact Ms. Greta Hamilton, Director, Marion National Cemetery, at 765-674-0284 to arrange a visit. A.GENERAL INFORMATION: The Department of Veterans Affairs, National Cemetery Administration intends to award a contract for arboricultural work. The primary emphasis during completion of the work shall be to improve the safety and appearance of the cemetery without compromising the safety of the public or cemetery personnel. All work must be done without disturbing or damaging existing gravesites, headstones, fences, buildings, or other cemetery property. Since this work is of a potentially dangerous nature, and requires special expertise, it is to be performed by a company that derives a majority of its annual income from arboricultural work and whose employees are highly trained and skilled in all phases of tree service work. The chosen contractor is required to follow the ANSI Z-133 Standards for tree worker safety. 1- The NCA Mission: The National Cemetery Administration honors veterans with a final resting place and lasting memorials that commemorate their service to our Nation. National cemeteries are national shrines. The standards of maintenance, appearance and operational procedures performed by the contractor at this cemetery shall reflect this nation's concern for those interred there. For this reason, the contractor's strict adherence to the specifications shall be required and shall be essential. B1.DETAILED SCOPE - REMOVALS: The contractor shall remove fourteen trees and one additional stump, as specified on the worklist. Removal shall constitute complete removal, including grinding of stumps to 8" below soil surface, removal of all surface roots, and filling the remaining holes with topsoil leveled to grade. Holes shall not be left open overnight. Wood chips shall not be used as backfill. All debris shall be promptly cleaned up, and disposed of off-site at contractor's expense. B2.DETAILED SCOPE - PRUNING - CROWN-CLEANING: The contractor shall crown-clean prune (remove damaged, dead, diseased, weakly attached, and hazardous limbs or branches) 119 trees as specified on the worklist. Trees shall not be topped. All pruning shall conform to the current edition of ANSI A300 (Part 1) Pruning standards and these specifications, and be done in a manner consistent with ISA Best Management Practices - Pruning - Second Edition. All pruning shall be done under the direct, on-site supervision of an ISA certified arborist. 1- All cuts shall be made as close as possible to the trunk or parent limb, without cutting into the branch collar or leaving a protruding stub, to ensure the greatest wound-wood (callus tissue) growth response to cover the wound. Bark at the edge of all pruning cuts should remain firmly attached. 2- All branches too large to support with one hand shall be precut to avoid splitting or tearing of the bark. All branches and/or tree material 3 1/2" in diameter shall be lowered by proper lowering methods such as using ropes. 3- The contractor shall prune and shape trees to retain (or restore, when needed) their natural crown characteristics and patterns as much as possible. 4- The use of climbing spurs or spiked shoes shall not be permitted on trees being pruned. 5- Use of equipment or practices that would damage bark or cambium beyond the scope of work shall not be allowed. This includes the use of wound paint (except to prevent spread of oak wilt). 6- Dead wood 25 mm (1 inch) or more in diameter, branches interfering with or hindering the healthy growth of the trees, and broken or diseased branches shall be removed with a clean cut. Branches are to be cut back as necessary to give the trees proper shape and balance. B3.DETAILED SCOPE - PRUNING - STRUCTURAL PRUNING: The contractor shall prune four trees, as specified on the worklist, to improve their structure, using appropriate removal and thinning cuts to remove multiple leaders, crossing branches, branches spaced too closely together, and poorly attached branches (narrow crotch angles and/or included bark). All pruning shall conform to the current edition of ANSI A300 (Part 1) Pruning standards and these specifications, and be done in a manner consistent with ISA Best Management Practices - Pruning - Second Edition. No more than 25% of the crown of any tree shall be removed. All pruning shall be done under the direct, on-site supervision of an ISA Certified Arborist. 1- All cuts shall be made as close as possible to the trunk or parent limb, without cutting into the branch collar or leaving a protruding stub to ensure the greatest wound-wood (callus tissue) growth response to cover the wound. Bark at the edge of all pruning cuts should remain firmly attached. 2- All branches too large to support with one hand shall be precut to avoid splitting or tearing of the bark. 3- The Contractor shall prune and shape trees to retain their natural crown characteristics and patterns as much as possible. 4- Use of equipment or practices that would damage bark or cambium beyond the scope of work shall not be allowed. Wound paint shall not be used. B4. DETAILED SCOPE - CABLING AND/OR BRACING: The eight trees specified on the worklist shall have support systems installed, specifically braces and/or cables (provided by the contractor), in conformance with ANSI A300 standards for tree supplemental support, and the ISA Best Management Practices publication "Tree Support Systems: Cabling, Bracing, Guying, and Propping, Third Edition." An ISA certified arborist shall evaluate each tree specified on the worklist, determine the best cabling/bracing appropriate for each, and install or direct the installation of cables/braces as appropriate to reduce risk of failure of the trees or their branches or leaders. B5. DETAILED SCOPE - WOUND SCRIBING: An International Society of Arboriculture certified arborist shall scribe or trace the wounds on three trees. Dead and injured bark shall be removed with a sharp knife or other appropriate tool, rounding the margins, and without enlarging the wounds. B6. LOCATING THE TREES: Trees to be removed or worked on are not physically marked. A detailed map is included with all trees accurately positioned relative to each other, and with roads and burial sections sketched in. On the map, trees requiring work are keyed according to work to be done, and are labeled according to their numbers on the work lists, in order numerically from south to north. The work lists also include the burial section number and a headstone or other feature very near each tree so it can be identified without error, which is essential. Directions are abbreviated on the work lists as N, S, E, and W for north, south, east, and west, and HS means headstone. Specific headstones are identified by their engraved numbers, or by name on the headstone if it has no number on it. C1.GENERAL PARAMETERS: 1- The contractor is responsible to ensure that all work shall be done in a manner that safeguards all VA visitors, employees, and public. The Contractor shall be solely responsible for any and all actions initiated and/or completed by his/her employees. Furthermore, the contractor and his/her employees shall be fully compliant with all applicable Federal, State, County and City laws, and ordinances. 2- Damage to Government property: Contractor shall be responsible for repair or replacement of any cemetery structure or property, including buildings, turf, curbs, road pavement, headstones or markers, water system components, or control markers, that are chipped, marred, damaged and/or ruined at the fault of the Contractor. The contractor shall bear all costs associated with replacement and reinstallation. Any such damage shall be brought to the immediate attention of the appointed COR prior to repair/replacement/installation. 3- Contractor shall be responsible for cleaning any cemetery structures that are soiled or stained as a result of contractor's performance. The contractor shall wash-down with water all soiled or stained structures and grounds at the end of each workday. No synthetic chemicals may be applied to Government property. The contractor shall bear all costs associated with washing and cleaning. Any such washing/cleaning shall be brought to the immediate attention of the COR prior to washing/cleaning. 4- At the end of each day the contractor shall remove all debris resulting from the work from the cemetery site. The contractor shall ensure at all times that rubbish and trash generated by the contractor is kept clear of vehicular and pedestrian traffic throughout the site. NOTE: The Government shall not provide receptacle(s) for disposal of debris related to this contract. 5- The contractor shall not operate trucks, tractors, and other heavy equipment on any turf or interment area except when authorized in writing by the COR. Spider-type lifts may be used on turf areas if the feet are adequately supported by plywood or boards to spread the weight distribution to prevent soil compaction and prevent depressions. 6- The contractor is responsible for the safety and security of his/her own equipment, tools, and supplies. The Government is not liable for damage, loss, or theft of contractors equipment, tools, and supplies. C.2CONTRACTOR-FURNISHED ITEMS 1- The contractor is responsible for supplying all equipment, personnel, tools, supplies and materials to perform these services. 2- Contractor-furnished items necessary to perform work as required under this contract shall be furnished, maintained and operated by the contractor and shall be consistent and fully compliant with all applicable Federal, State, County, City laws, ordinances and regulations. 3- The contractor is responsible for ensuring that all of his/her motor vehicles and equipment meet State's inspection, safety, licensing, registration, and insurance requirements. C.3CONTRACTOR CONDUCT 1- Due to the sensitive mission of the cemetery, the work often requires contact with, and exposure to, grieving individuals. Contractor personnel must exercise and exhibit absolute decorum, composure, respect, and stability at all times. Inquiries from cemetery visitors shall be politely referred to cemetery staff. 2- Contractor shall maintain a neat and professional appearance throughout its workforce, vehicles, equipment, and maintenance areas. 3- Contractor employees shall not engage in loud or boisterous behavior, angry outbursts or use profane or abusive language at anytime on Government premises. Playing radios and/or electronic games/devices, smoking or chewing tobacco products, shall only be done at lunchtime and in a designated work area. C.4SAFETY: Matters related to safety, and any actions of the contractor, must meet all safety requirements of OSHA, the State, and ANSI Standard Z133. It is incumbent upon the contractor to be familiar with these requirements. C.5SUBMITTALS: Licenses/Certification: Copies of certificates of liability, worker's compensation insurance, and arborist's certification will be required before work can begin. The contractor shall comply with all State, Local and Federal licenses/certifications required to perform the work. C.6RESTORATION AND CLEAN UP: Remove debris and waste material daily from the cemetery. In areas where work has been completed, clear the area of all debris, spoil piles and containers. Keep at least one paved vehicular access route to each burial section clean at all times. C.7UTILITY LINES COMPLIANCE Where pruning/stump grinding/tree removal conflicts with existing utility/service lines (above ground/below ground), the corresponding utility company shall be notified by the contractor. The contractor shall obtain any necessary permits/ blue prints and cooperate with the utilities company/cemetery staff to avoid any damage or liability, and provide a safe work environment for his/her employees. C.8PERFORMANCE PERIOD: The contractor shall complete the work required under this statement of work in 75 calendar days or less from the date of award, unless otherwise directed by the contracting officer. C.9WORK HOURS: All work shall be performed during the normal working hours (8:00 a.m. to 4:30 p.m.), Monday through Friday, except with the written permission of the COR or his/her representative. When emergency situations are caused by the contractor, then he/she shall make arrangements with the COR to work on weekends to correct/eliminate the emergency in order to meet the period of performance. Work at the cemetery shall not take place on Federal holidays. The Government shall not compensate the contractor for emergency situations caused by the contractor. C.10DAILY SCHEDULE: The contractor shall coordinate with the COR daily for the purpose of establishing a work schedule and to ensure that no work is being performed at the immediate site of a scheduled interment or ceremony. A list of scheduled ceremonies will be provided the week prior to the event, and a list of scheduled funerals will be provided the day prior to the service. The contractor is solely responsible for ensuring that no contract work causes any funeral, ceremony, procession or visitation to be delayed, altered, or otherwise impacted in such a way that the dignity or security of the event is compromised. The contractor is solely responsible for staying abreast of all such upcoming events and when in doubt, he/she must ask the COR. C.11RECORD KEEPING AND REPORTING: The contractor's site manager or his designee shall provide the COR with weekly written accurate reports identifying work completed during the previous week. The site manager will also provide upon request a plan of action for the upcoming week. ?
