- Existing site plan at bypass channel Amendment #4, 1) "What is the time/place of the pre-bid meeting?" -The pre-bid meeting will be held on Tuesday June 23rd at 1:00pm. It will be located at the Knutson Dam project site. 2) "The existing contours in the box culvert area are not shown in locations where new contours are required. We will need that information to accurately determine the amount of grading required." -A new plan sheet #10A, "Existing Site Plan at Bypass Channel", is attached for addition to the drawings by amendment. Amendment #3, On Page 3 of the solicitation, B-2. Notes: The correct phone number for the Engineer Craig Taylor is (218) 335-8682 Amendment #2, 52.228-1 -- Bid Guarantee. Bid Guarantee (Sep 1996) (a) Failure to furnish a bid guarantee in the proper form and amount, by the time set for opening of bids, may be cause for rejection of the bid. (b) The bidder shall furnish a bid guarantee in the form of a firm commitment, e.g., bid bond supported by good and sufficient surety or sureties acceptable to the Government, postal money order, certified check, cashier's check, irrevocable letter of credit, or, under Treasury Department regulations, certain bonds or notes of the United States. The Contracting Officer will return bid guarantees, other than bid bonds -- (1) To unsuccessful bidders as soon as practicable after the opening of bids; and (2) To the successful bidder upon execution of contractual documents and bonds (including any necessary coinsurance or reinsurance agreements), as required by the bid as accepted. (c) The amount of the bid guarantee shall be _20_ percent of the bid price or $200,000.00, whichever is less. (d) If the successful bidder, upon acceptance of its bid by the Government within the period specified for acceptance, fails to execute all contractual documents or furnish executed bond(s) within 10 days after receipt of the forms by the bidder, the Contracting Officer may terminate the contract for default. (e) In the event the contract is terminated for default, the bidder is liable for any cost of acquiring the work that exceeds the amount of its bid, and the bid guarantee is available to offset the difference. (End of Provision) Perofrmance bond will be required at time of award (Standard Form 25), 100% of the original contract price. Amendment #1, Full Solicitation added 5 June, 2015 The USDA Forest Service, Chippewa National Forest, located in Cass Lake Minnesota anticipates issuing a Request for Proposals to demolish Knutson Dam and replace it with a rock arch rapids. The project is located on the Mississippi River at the outlet of Cass Lake, on the Blackduck Ranger District (Beltrami County, MN) on the Chippewa National Forest. The contractor(s) shall provide all expertise, labor, supervision, services, equipment, tools, supplies, and permits (when and if required), transportation, and incidentals necessary to perform the dam demolition and rock arch rapids construction along with other associated minor activities. Work includes, but is not limited to site dewatering, construction of a bypass channel including installation of a concrete box culvert, demolition and removal of the existing dam, earth excavation and filling, construction of the rock arch rapids, installing shoreline riprap, and installing a "T" shaped fishing pier. Work will be completed late in the 2015 construction season. Price range is between $500,000 and $1,000,000 with options. The solicitation and related documents will ONLY be available for download at http://fbo.gov/ on or about June 5, 2015. No hard copies of the solicitation will be mailed out. Interested vendors should register to receive notifications through this site. In order to receive award under this RFP the successful offeror shall be registered in the System for Award Management (SAM). NAICS: 238910, the size standard is $14 Million. This project is being solicited as a 100% set aside for Small Business. All work is to comply with local, Federal and State codes and is subject to Department of Labor wage rates. Solicitation Number AG-6392-S-15-0015 All responsible sources may submit a proposal which shall be considered by the Forest Service. This is a synopsis, the full solicitation documents including specifications will be issued on or about June 5, 2015. Deadline for proposals will be July 6, 2015.
