Description
STATEMENT OF WORK VA NORTHAMPTON FRANKLIN COMMUNITY BASED OUTPATIENT CLINIC (CBOC) LAB WORK AND SPECIMEN COLLECTION SERVICES DEFINITIONS Stat Test- A test required to be performed within 3 miles of the VA Greenfield CBOC and processed within 60 minutes of collection that is specially handled, tested and reported, within the specified turn-around-time. TAT- The length of elapsed time between collection of the specimen from the contractor's laboratory until the receipt of the completed printed report back in the clinic. Test Result Report - A printed final copy of laboratory testing results. Critical (Panic) Value- Those test results that require evaluation by a physician or other health care provider as soon as verified. Failure to take appropriate action as a result of critical value might cause harm to a patient. Proficiency Testing- An assessment of the accuracy of testing by a laboratory based on the analysis of an unknown specimen analyzed by a large number of other labs. The proficiency survey is conducted by an organization or agency authorized by the Department of Health and Human Services to do so. VA Greenfield Outpatient Clinic(CBOC) - 143 Munson St., Greenfield Corporate Center, Greenfield, MA 01301-9660, Tel: 413-773-8428, Fax: 413-773-8435 VA Medical Center - VA Northampton, 421 N. Main St, Leeds, MA 01053-9764 VistA- VA Medical Center computer system The contractor must provide the following services: 1.SPECIMEN COLLECTION SERVICES A.The Contractor will provide phlebotomy services as well as collection of urine specimens as needed to patients presenting with orders from the CBOC. B.The CBOC will provide FMC with the NCL User's Manual which outlines specimen collection guidelines for specimens to be tested at the VA Medical Center. C.The Contractor will process specimens to the extent needed to preserve specimen integrity (i.e. centrifugation, pour off). D.The Contractor will provide a daily record of blood and urine specimens being shipped to the VA Medical Center for tracking purposes. List will be included with shipment or Faxed. Fax number: (413)582-3054. E.The Contractor will accept and package for pick up by VA Contracted Courier all routine specimens collected in addition to any PAP, Occult Blood or other specimen collected on site (urine cups are provided by the Northampton VA Lab) or delivered to FMC for transport to the VA Medical Center or its Reference Laboratories. A "VA Northampton" label must be placed on each specimen bag. Additional bags and labels may be obtained by calling Quest Diagnostics Client Services at 617-547-8900. Pickup will be made at or around 4pm Monday through Friday (excluding federal holidays). F.Packaging of specimens will be according to the protocols established between the VA Medical Center and Quest Diagnostics (or current reference laboratory). G.The Contractor will perform limited STAT testing (refer to Attachment A) for VA patients presenting with valid orders from a VA clinician. H.The Contractor will keep a log of patients served and forward a copy of this log to the VA Medical Center Laboratory Manager monthly for billing reconciliation. 2.ANALYSIS OF SPECIMENS FOR STAT TESTING A.Analyze samples: Contractor is required to perform STAT analytical testing for patients presenting at FMC for the tests defined on the STAT list in Attachment A. The Contractor is required to provide a reference test manual, and report of analytical test results and consultative services as required assimilating the full scope of its laboratory operations. B.The Contractor will consult the VA Medical Center on test results by telephone as needed. C.Reporting of STAT Results: 1.STAT test results will be called to the ordering physician or backup as listed in Attachment A within 60 minutes of specimen receipt by the contractor. 2.Critical test results will be called to the ordering physician or backup as listed in Attachment A immediately within 20 minutes of the result being deemed acceptable by the contractor. 3.Test result reports shall be faxed to the clerical office of the VA Medical Center for entry into the medical record. The report at minimum will contain the following information: "Patient's full name, "Patient's full social security number or unique hospital identification number " physician's name, "date/time of specimen collection, "date test completed, "test name, "test result, "flag abnormal results, "reference range, "testing laboratory testing number (accession number), "type of specimen, "any additional comments related to test provided by ordering physician, "any other information the laboratory has that may indicate a questionable validity of test results, "Unsatisfactory specimen shall be reported with regard to its unsuitability for testing. 3.LICENSING AND ACCREDITATION- The Contractor A.Shall have all licenses, permits, accreditation certificates required by law. B.Shall be accredited by the College of American Pathologists (CAP). C.Laboratory Director is a licensed Physician or a licensed Bioanalyst. D.Must comply with the regulatory requirements of Centers for Medicare and Medicaid. 4.QUALITY CONTROL- To ensure proper handling and test performance, the contractor shall provide the following updated information upon request during the life of the contract: A.For quality control purposes, tests routinely performed in duplicate shall be indicated. B.Coefficient of variation of quality control samples of all tests or specified tests the laboratory performs. C.Proficiency testing data provided to customer will include where challenges failed, a list of tests outside of the +/- 2SD range for the past (2) years. Contractor shall notify VA Medical Center of any test outside +/- 2 SD range during contract period. D.The VA Medical Center will maintain an Internal Quality Control Program to monitor the quality of test results received from the contractor. The method used for monitoring is at the discretion of the VA Medical Center and may include, but is not limited to, unidentified split specimens sent periodically to the contractor for testing, split specimen sent to another reference laboratory for comparison, or monitoring of turn around time. 5.The contractor's facilities, methodologies (defined as the principal of the method and the references), and quality control procedures may be examined by representatives of the VA at any time during the life of the contract. 6.SERVICE: A.The contractor shall provide a Laboratory Manual containing the following information: "Department hours of operation "Accreditation "Technical Staff "Service Departments( Method of contacting, phone numbers, hours of availability) "Quality Assurance Information "Billing Procedures & fee schedules for services provided "Procedures and criteria for phoning reports and other important information "Report forms B.The contractor shall provide telephone number(s) and contact person(s) to be used by the VA Medical Center to make specimen problem inquiries and problem solving at all times including weekends and holidays. *NOTE: Also include names and telephone number(s) of technical Directors and Pathologists available for consultation. C.Contractor agrees to maintain the minimum acceptable service, reporting systems, and quality control as specified herein. Immediate (within 24 hours) notification must be given to VA upon adverse action by a regulatory agency. D.Contractor shall assign a specific local account representative. E.Contractor shall advise facility of any changes in methodology, procedure or reference ranges. 7.TELECOMMUNICATIONS A.The VA lab shall receive transmission of STAT test results to the fax machine located in the VA Medical Center clerical area. Fax number is 413-582-3054. Critical values and STAT results are called to the CBOC ordering physician or backup as listed in Attachment A. When the CBOC is closed the Critical values and STAT results are called to the VA Medical Center MOD or backup as listed in Attachment A. 8.TESTING METHODOLOGY- Testing methodology for a test must be defined in the contactor's laboratory users manual. 9.BENCHMARK REFERENCE- VA Medical Center may submit a number of specimens for analysis as a benchmark reference. These tests shall be performed and reported without charge to the VA. 10.FEE SCHEDULE: Payment shall be at standard Medicare rates for all work performed. 11.PAYMENT A.Payments shall be made via 2237(certified invoice). Payment shall be processed thirty (30) days from receipt of proper invoice. Invoice shall include patient name, DOB, ss#, date of and service provided.
