Description
STATEMENT OF WORK Syracuse VA Medical Center 800 Irving Avenue Syracuse, New York 13210-2716 1.All equipment is located at Syracuse VA Medical Center, unless otherwise noted. 2.Contractor shall furnish all tools, labor, material, transportation and parts necessary to maintain equipment in fully operating condition. Contractor shall use only OEM Original Equipment Manufacturers parts. Substitution parts will invalidate FDA OK Requirement and the equipment will be considered non- fully operational. 3.Contractor shall provide PM services schedule for all equipment listed in the schedule. 4.Contractor shall maintain equipment in proper operating condition to assure optimum performance and reliability. Contractor to provide all labor for equipment inspection, adjustment, lubrication, installation of OEM replacement parts and testing during scheduled preventive maintenance (PM) inspections. 5.Contractor shall respond to requests for breakdown repairs "same day" within four (4) hours of verbal request by authorized VAMC personnel (Engineering and CS) per the Delegation of Authority furnished to the successful Contractor. 6.Contractor shall perform preventive maintenance (PM) inspections semi-annually (every 120 days) and provide full coverage to include parts, labor and travel in accordance with established standard industry procedures. 7.Contractor shall ensure that PM inspections are conducted by factory trained service personnel. 8.Contractor shall provide proof of Factory (OEM) Training documentation for each specific Model of equipment to be serviced. 9.Contractor shall provide documentation showing all personnel are trained in Infection Control Practices, Blood Bourne Pathogens Patient Rights and Confidentiality (HIPPA). 10.Further Documentation must assure that the service provider has passed a Back ground check and Drug Screen. 11.Al l service performed upon the equipment must assure compliance with Section 510K of the Food, Drug, and Cosmetic Act and in accordance with the standards philosophy of the Association for the Advancement of Medical Instrumentation. 12.Service provider must be ISO 9000 and 13485 Certification. 13.Service provider must use and provide documentation for calibrated and traceable testing equipment, such as Electrical, Pressure, Vacuum, and Temperature measuring devices. 14.Service provider must provide in writing an escalation policy containing the support structure of the service company to include, parts search capability, engineering support and high level field support. 15.Service provider must supply proof of software to communicate with equipment software for the purposes of programming and troubleshooting. 16.Service Provider must maintain a parts consignment on-site of those parts and parts rebuild packages that are necessary to meet 90% of the most common type of equipment failures. Other parts must be brought in on an as needed basis using the standard modes of transportation from the parts vendor/distributor. 17.After each service call, Contractor shall provide the SPS Equipment Specialist, Engineering Service with a service call or preventive maintenance report. To include: a.Machine model and serial number b.Date, time called and time of arrival c.Work performed, parts replaced (etc.) d.Reason for service call 18.Contractor shall submit a copy of preventive maintenance procedures conducted to assist the Medical Center in meeting VA's requirements. 19.Equipment Information: STERRAD ®100NX 1-DR, Serial Number 1041110560. 20.Hours of Coverage: A. Normal business hours are: Monday through Friday from 7:30 am to 4:00 pm, excluding Federal holidays as listed in paragraph C below. All service/repairs will be performed during normal business hours of coverage requested and approved by the Contracting Officers Technical Representative (COR). Contractor's representative shall check in with Engineering upon entering the facility and shall sign in and out of the facility with Engineering. B. Preventative maintenance inspections will be performed at a time/date mutually acceptable to Engineering, the clinical users, and the Contractor. C. Federal Holidays observed are: New Years DayLabor DayMartin Luther King Day Columbus DayPresidents DayVeterans Day Memorial DayThanksgiving DayIndependence Day Christmas Day Any other day specifically declared by the President of the United States to be a National holiday. 21.Wage Determination: U.S. Department of Labor Wage Determination: In compliance with Section IV, Contract Clauses, incorporating the provisions of FAR 52.222-41 Service Contract Act of 1965, and FAR Clause 52.222-42, Statement of Equivalent Rates for Federal Hires, offerors are advised that service workers employed under any contract resulting from this solicitation shall be paid not less than the minimum amounts shown in the U.S. Department of Labor Wage Determination available at: http://www.wdol.gov/.
