THIS IS A SOURCES SOUGHT NOTICE INTENDED FOR MARKET RESEARCH PURPOSES ONLY. There is no solicitation available at this time. No contract will be awarded from this announcement. The purpose of this Sources Sought Notice is to determine the availability and capability of the commercial market for Preventive Maintenance service for our Hemodialysis Water Purification Equipment. This notice is for planning purposes only, and does not constitute an Invitation for Bids, Request for Proposals, or Request for Quotations, or an indication the Government will contract for the items contained herein. This notice is NOT to be construed as a commitment on the part of the Government to award a contract, nor does the Government intend to pay for any information submitted as a result of this notice. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The VA Healthcare System of Ohio Network Contracting Office (NCO) 10 is conducting market research to identify potential qualified sources to provide Preventive Maintenance service for our Hemodialysis Water Purification Equipment. The attached statement of work is provided for review and edification. If interested please submit your response(s) to Adoniss.Gibson@va.gov. Department of Veterans Affairs NCO 10 Contracting - NBC 4100 West Third Street Dayton, OH 45428 STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project: Preventive Maintenance for Hemodialysis Water Purification Equipment annual service contract 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. a.Planned Maintenance Agreement for Hemodialysis Water Purification Equipment: i.Contractor to perform monthly preventive maintenance service visits where all operational conditions are logged and verify the efficiency to assure proper system operation. Evaluate membrane performance to determine if replacement is necessary. Discuss any recommendations or changes with customer. Send results of readings and tests with recommendations. ii.Replace worn or failed parts (parts not included in PMs will only be replaced upon approval of the VA Medical Center), as needed during the monthly preventive maintenance (PM). iii.Take monthly LAL samples which will include Pre and Post dialysis loop and will pick-up (2) dialysis machine samples. This will be a total of (4) per month. Additional LAL test will be at current pricing. Result report of these tests must be provided to the Dayton VA. iv.Disinfection of system will be monthly. v.Visit the VA Center and log and inspect the water treatment system. vi.Perform system checks on all components. vii.Verify the efficiency to assure proper system operation. viii.Exchange system and RO prefilter cartridges provided by the VA Medical Center. ix.Evaluate membrane performance to determine if replacement is necessary. x.Discuss any recommendations or changes. xi.Send results of readings and tests with any recommendations. xii.Take AAMI sample every six months due in June and December each year and send report to VA. Ensure all AAMI required testing is done at the recommended intervals. On-site facility staff training annually if required. xiii.Six (6) model 45844 deionizer exchange tanks in worker/polisher arrangement, rotate on exchange with three (3) tanks exchanged as needed. Ultra pure, type one mixed bed resin is utilized. Normal exchange is next business day unless it is needed sooner. DI tanks used in excess of twenty-seven (27) are not covered by this contract. Additional tanks will be billed at $145 each. xiv.Two (2) model 111447 carbon tanks in a worker/polisher arrangement, rotate on exchange with one (3) tanks every three months, or as needed. Carbon tanks in excess of fifteen (15) per year are not covered under this agreement. Additional tanks will be billed at $348 each. b.Planned maintenance agreement to be performed on the Deionizer Exchange Service in the laboratory: i.Two-part number 451447 Ultra-Pure Deionizer Tanks in a worker/polisher configuration. One (1) tank is exchanged as needed. Normal exchange is next business day unless it is needed sooner. DI tanks used in excess of fifteen (15) are not covered by this contract. Additional tanks will be billed at $190 each. ii.Provide Model 20-5101 carbon block pre-filter to be replaced each time deionizer tank is exchanged. Filters in excess of fourteen (14) are not covered by this contract. Additional filters will be billed at current price. iii.Provide model 20-3021 submicron post filter and replace every six (6) months. Filter used in excess of three (3) are not covered by this contract. Additional filter will be billed at current pricing. 3.Background: We currently have a contract on the hemodialysis water purification equipment and lab. This contract expires September 30, 2015. 4.Performance Period: The contractor shall complete the work required under this SOW from October 1, 2015 - September 30, 2016, unless otherwise directed by the Contracting Officer (CO). Four option years from October 1, 2016 - September 30, 2017, October 1, 2017- September 30, 2018, October 1, 2018 - September 30, 2019, and October 1, 2019 - September 30, 2020 5.Type of Contract: Firm Fixed Price 6.Conformance Standards: Contract service shall ensure that the equipment functions in conformance with the latest published edition of NFPA-99, UL, OSHA, and VA. 7.Operational Uptime - Requirements: The water systems shall be operable and available for use 99% of the normal operating hours of the equipment. Downtime will be computed from notification of problem during normal work hours. Scheduled maintenance will be excluded from downtime. Normal work hours are 8:00 a.m. to 4:30 p.m., Monday thru Friday, excluding national holidays. Operational Uptime will be computed during a month long time period. Repeated failure to meet this requirement can subject the contractor to DEFAULT action. 8.Hours of Coverage: a.Normal hours of coverage are Monday through Friday from 8:00 am to 4:30 pm, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR. b.Work performed outside the normal hours of coverage at the request of COR will be billed at no additional charge to the Government. NOTE: - Hardware/software update/upgrade installations will be scheduled and performed outside normal hours of coverage at no additional charge to the Government (unless it would be detrimental to equipment up-time; to be determined by the COR). c.Federal Holidays observed by the VAMC are: i.New Years' Day ii.Martin Luther King Day iii.Presidents' Day iv.Memorial Day v.Independence Day vi.Labor Day vii.Columbus Day viii.Veterans' Day ix.Thanksgiving Day x.Christmas Day 9.Check in and check out requirements: The Contractor shall be required to report to Biomedical Engineering to log in. This check in is mandatory. When the service is completed, the FSE shall document services rendered on a legible Service Report(s). The FSE shall be required to log out with Biomedical Engineering and submit the Service report(s) to the COR. ALL service reports shall be submitted to the equipment user for an "acceptance signature" and to the COR for an "authorization signature". If the COR is unavailable, a signed, authorized copy of the service report will be sent to the Contractor after the work can be reviewed (if requested or noted on the service report). Service report shall describe the work that was completed and list parts that were replaced. A copy of the service report shall be signed and left with a representative of Biomedical Engineering prior to the departure of the service person. The reports with the LAL and AAMI test results shall be sent as soon as possible to: Biomedical Engineering (138G) VA Medical Center 4100 West Third Street Dayton, OH. 45428 VA Biomedical Contact Person(s): ___________ Location (Building/Room Number): Building 330/Room BC123______ Telephone Number & Extension(s): ____________ Contractor - Provide telephone number(s) to call for your Service Department: ______ Provide name(s) of authorized contact person(s): ______ 10.Emergency service visits: The DVAMC will provide first-call screening to determine if there is a legitimate equipment problem. Upon determination of a legitimate equipment problem, the DVAMC will contact the Contractor to arrange a service call. The contractor shall respond by telephone within two hours, and shall arrive at the Medical Center within twenty-four hours to service the equipment. The contractor shall perform all work necessary to assure that the equipment meets or exceeds manufacturer's specifications. 11.Preventive maintenance inspections: Preventative Maintenance Inspections (PMI's) shall be performed quarterly per original equipment manufacturer's specifications. The inspections will be scheduled at a mutually agreed upon time. The PMIs may include but are not limited to calibration, cleaning, lubrication and adjustments. 12.Competency of personnel: All work shall be performed by competent technicians, experienced and qualified to work on the equipment listed in Section B. All personnel servicing this equipment must have had formal training. In accordance with Section M, offeror's shall submit with their technician proposal, the names and experience of the primary/back-up personnel who will perform work under this contract. Any substitution of service personnel must have prior approval of the Contracting Officer except in emergencies where retroactive approval can be sought. All work performed shall be in accordance with commercial practices and the equipment manufacturer's specifications. 13.Documentation: For all service visits, the Contractor shall furnish a service report describing the malfunction and all work and materials required to correct the problem. 14.Acceptance of work: Completion of all work is subject to the approval of both the Clinical Service Chief and the Biomedical Engineering Section personnel. In addition, the documentation of the work is subject to approval by both representatives of both services. When the work is deemed satisfactory and documentation is provided, representatives of both services will indicate their approval on the Contractor's service sheets prior to the Contractor's departure. Repair, damage or malfunctions caused by physical damage, massive power failure accidents, negligence, misuse or abuse, weather conditions or any other factor not associated with the reasonable and normal use of the equipment or originating outside of the equipment is not required of the contractor. During the period in which this agreement is in effect, no personnel other than employees of the contractor or DVAMC Biomedical Engineering Section shall perform service or maintenance on the equipment unless specifically authorized by the Contractor. 15.Safety requirements: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer or his/her designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer or his/her designee will notify the contractor of and noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when serviced, on the contractor or his representative at the site of the work, shall be deemed sufficient for the appropriate action. If the contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. 16.Condition of equipment: The Contractor accepts responsibility for the equipment in "as is" condition. Failure to inspect the equipment prior to contract award will not relieve the Contractor from performance of the requirements of this contract. 17.Identification, parking, smoking, and VA regulations: The Contractor's FSEs shall wear visible identification at all times while on the premises of the VAMC. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings at the VAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. B. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. C.ADMINISTRATIVE REQUIREMENTS FOR CONTRACTOR EMPLOYEE COMPETENCIES 1.The Contractor will be responsible to ensure that Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. 2.The Contractor is required to maintain records that document competence/performance level of Contractor employees working on this contract in accordance with JCAHO and other regulatory body requirements. 3.Not withstanding other contract requirements, upon request of the Contracting Officer, the Contractor will remove from the work site, any Contractor employee who does not comply with or meet competency requirements for the work being performed. 4.When changes in Contractor personnel occur, the Contractor must provide evidence of the current competence assessment and current performance evaluation. D.QUALIFICATIONS: Offers will be considered only from offerors who are regularly established in the business called for and who, in the judgment of the Contracting Officer, are financially responsible and able to show evidence of their responsibility, ability, experience, equipment, facilities, and personnel directly employed or supervised by them to render prompt and satisfactory service in the volume required for all items under this contract by the signing of this offer, offeror is certifying that he/she shall meet all requirements of Federal, State, or local laws, codes, and/or regulations regarding the operation of this type of service.
