Description
The Defense Health Agency (DHA) Contracting Operations Division - National Capitol Region, Falls Church, VA is seeking sources to provide all necessary and required Peritoneal Dialysis (PD) supplies and equipment to the patients of the Department of Nephrology, Walter Reed National Military Medical Center (WRNMMC) who are in treatment for End Stage Renal Disease (ESRD). The vendor is required to provide all PD supplies and equipment to WRNMMC's peritoneal dialysis patients using the. The peritoneal dialysis home treatment modality supplies and equipment shall be provided only as prescribed by the patient's physician and reflective of the current standard of care for such patients. Since the patients are, and will continue to be self-dialyzing under the peritoneal dialysis home modality; the vendor will be responsible for; and, facilitating of; the delivery of all supplies directly to the patient, at their homes; or other, temporary residences. The vendor is to also coordinate vacation delivery supply requests, at no supplemental, additional expense to the government for shipments within the continental US. Any shipments outside the continental US that might incur unusual or extraordinary additional costs; are to be advised to the Government in advance of shipment, and specifically approved prior to such shipment being authorized for release. The vendor will be required to coordinate, and be responsible for off-schedule or emergency delivery requests; and will be responsible for coordination of overnight/urgent deliveries of supplies based patients' Physicians initiating a prescription; or, changing of an existing Physician's prescription; at no additional or supplementary cost to the government. The vendor will provide PD supplies/equipment typically on a 28 day delivery schedule one prescription to each patient on the PD modality. This will usually result in approximately thirteen (13) deliveries per patient, annually. There are currently six (6) patients receiving supplies and equipment; but over the course of the intended contract period (one base year; and four (4) Option Years) PD patients could increase up to ten (10) patients. The vendor shall be responsible for disposal of all used and unused equipment and supplies, in accordance with State laws and regulation regarding Regulated Medical Wastes (RMW). The peritoneal dialysis (PD) supplies and equipment provided to WRNMMC's peritoneal dialysis patients must be identical in their use to that of the existing PD home treatment modality; and, must have the same or equal Physician and patient operational familiarity, training regimen, and supply commonality, as with the existing PD home treatment modality. The North American Industry Classification System (NAICS) for this requirement is 446199; and the Small Business size standard is $7.5 million dollars. All responsible sources may submit a response. A determination made by the Government to not compete this requirement will be based on responses to this notice and this decision will be solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether, to conduct a competitive procurement. A solicitation will not be published. Interested firms may identify their interest regarding this notice, in writing to the Government point of contact. All responses to this notice must be received no later than 10:00 AM EDT, 08 July 2015. Telephone or facsimile inquiries are not acceptable in response to this notice.
