This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-15-Q-0596 is issued as a (Request for Quotation (RFQ). This requirement is Small Business. NAICS: The NAICS is 335999 and the small business size standard is 500 employees. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. To be receiving a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. Offeror may register with CCR at www.sam.gov Scope of Work: PART I - GENERAL 1. SCOPE: The firm shall provide nine surge protection devices as outlined in the Description of Supplies below. This equipment is part of the recommendations from the 2014 Electrical Coordination & Reliability Enhancement Study (ECRES) on the campus of the James H. Quillen VA Medical Center in Mountain Home, TN (Johnson City, TN). Summary of required equipment outlined. Phase1: 1. 5 devices to protect 120Y/208V panels rated up to 400 amps. 2. 4 devices to protect 277Y/480v or 480V panels rated up to 400 amps. 2. REQUIRED DOCUMENTS: All equipment will include the manuals, instruction books etc ¦.. Items required per scope Part 1 (1): All items are to be provided in digital format on CD or via email where applicable. 3. Provide equipment for 9 panel surge suppression devices. The facility intends to standardize on typical device used for surge protection at selected panels. The following denotes the line that has been used on a previous project. (5) SOLA CAT. NO. STV400K-10Y, SUB-PANEL SURGE PROTECTION DEVICE - FOR 120Y/208 VOLT PANELS (4) SOLA CAT. NO. STV400K-27Y, SUB-PANEL SURGE PROTECTION DEVICE - FOR 277Y/480 VOLT PANELS All equipment will adhere to all applicable codes and standards including but not limited to all applicable requirements of NFPA, National Electric Code, National Electrical Safety Code (NESC), Life Safety Code, OSHA, IEEE, VA Specifications, VA Standard Details, VA Construction Standards and Criteria, and applicable local, state and federal regulations. Description of Supplies: The VHA NCO 09 - Murfreesboro, TN requires the following items, Request for Quotes are for products that are an exact match to the following: LI 001: SUPPRESSOR; Product Range:STV 200/400K Series; Suppressor Type:Transient Voltage; No. of Outlets:-; Outlet Type:-; Voltage Rating:208V; Output Current:15A; Peak Surge Current:400kA; Response Time:500ps; Plug Type:-; Attenuation:50dB 5EA. LI 002: Plug Type:-; Product Range:STV 200/400K Series; Suppressor Type:Transient Voltage; No. of Outlets:-; Outlet Type:-; Voltage Rating:480VAC; Output Current:15A; Peak Surge Current:200kA; Response Time:500ps ;RoHS Compliant: No 4EA. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation. To receive consideration as SDVOSB or VOSB, the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document.
