- W-9, Self-Certification Form, Vendor Set-Up Form MTC's Terms and Conditions Service Contract Act Wage Determination Request for Quote THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Wind River Job Corps Center, referred to henceforth as the Center. The Center is requesting bids for a one (1) scissor dock lift and one (1) fork lift as referenced in the Statement of Work below.
