- Attachment A SOURCES SOUGHT NOTICE: PIMC-2015Q This is not a Request for Proposal (RFP) or Request for Quotation (RFQ). This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Indian Health Service (IHS), Phoenix Indian Medical Center (PIMC) requires the expansion of the existing Hologic SecurXchange PACS memory to allow more 3D digital images to be quickly retrieved by the associated Hologic SecureView viewing station (Refer to attachment A for additional information). Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 423430 and sized standard in employees is 100. NOTE: Respondent claiming Small Business shall check their social economic status or check Large Business. Anticipated period of performance: One base year and four one year options, total of five years. Other important considerations: Respondents shall provide information on service plan available for the equipment listed and provide plan description. If more than one plan, provide plan description. Capability statement /information sought: Response to this announcement shall not to exceed 5 pages and should include the following information: 1. A tailored capability statement indicating the firm's ability to provide the product and related services 2. The respondents' DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code and government contract number if applicable. 3. Information should be provided electronically in a Microsoft Word or Adobe PDF format. Please e-mail your response with the above information to Bradley.Platero@ihs.gov with the subject line "Sources Sought Notice #15-247-SOL-00108) by July 13, 2015, 12:00 PM MST.
