Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The commercial items procedures of FAR Part 12 will be used. NAICS code is 339112, size standard 500 employees. In accordance with VAAR 819.502-2, this action is a total small business set-aside. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76 effective July 25, 2014. This acquisition is being conducted under the authority of the commercial item program at FAR 13.5. Only electronic offers will be accepted. Offers are due tochristopher.blazejewski@va.gov by 12:00 PM. Eastern Time, July 9, 2015. All responding vendors shall be registered in SAM and have completed Representations and Certifications in accordance with FAR 52.212-3. The Department of Veterans Affairs, VA Medical Center, located in Cleveland, OH, has a requirement for Humphrey Field Analyzer. New equipment only, all items must be quoted, brand name (Zeiss) or equal, low cost technically acceptable. F.O.B. Destination preferred. See schedule. Place of Performance Department of Veterans Affairs Louis Stokes VA Medical Center Attn: Optometry/Bio Med 10701 East Boulevard Cleveland, OH 44106 The Humphrey Visual Field Analyzer is an integral piece of instrumentation found in all VA VISN 10 Medical Center Eye Clinics. The current visual field units in all outpatient clinics that offer Optometry are older models and are in need of replacement. The 750i Humphrey Field Analyzer will be used to replace the current Humphrey Visual Field in all Optometry clinics. In addition, Mansfield will need 1 Zeiss Humphrey FDT Visual Field Screener. Eight (8) Zeiss 750i HFA II should be purchased and one (1) Zeiss Humphrey FDT. Equipment to be delivered to VAMC Cleveland, Wade Park campus warehouse. Once delivered to the warehouse, they will be inspected by BioMed and then tagged with an EE number. Once it is tagged with an EE number, HFAs will be delivered to each CBOC. Requirements: There are a total of Eight (8) CBOCs who will require a 750i Humphrey Field Analyzer II to be purchased for each clinic to be used in their Optometry department. Must come with printer and table. Must be able to ensure Dicom connectivity. Must have the ability to receive a consult from CPRS and transmit the images to VISTA to be stored. Also must come with a slider for wheel chair. Mansfield CBOC also requires 1 Zeiss Humphrey FDT Visual Field Screener to be purchased. Eight (8) Zeiss 750i HFA II should be purchased and one (1) Zeiss Humphrey FDT. Equipment to be delivered to VAMC Cleveland, Wade Park campus warehouse. Once delivered to the warehouse, they will be inspected by BioMed and then tagged with an EE number. Once it is tagged with an EE number, HFAs will be delivered to each CBOC. See attached.
