Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. SAO West NCO 18 is planning to solicit for a vendor who can provide autopsy services off-site of the Southern Arizona VA Health Care System (SAVAHCS). SAO West NCO 18 is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this acquisition. Contractor must be able to meet all requirements of the Performance Work Statement. Interested parties shall provide a brief overview of their firm's capabilities and past experience with projects similar in scope to this project. Capability statements shall include: Copy of training certificates for each individual assigned to provide services. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: a. DUNS number b. Organization Name c. Organization Address d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size f. Socio-economic status ( IF SDVOSB or VOSB - provide SDVOSB or VOSB CVE verification) Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 621111. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than: July 17, 2015, 5:00 PM, ET. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a combined synopsis/solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality: No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). Interested parties may register at http://www.fedbizopps.gov to receive notification when the solicitation and any amendments are issued and available for downloading. We recommend that you monitor the FedBizOpps site for all information relevant to desired acquisitions. Background: This contract is for provision of autopsy services for deceased SAVAHCS patients as requested/required by family members and/or SAVAHCS provider or pathologist. Scope: The scope of this contract is as follows: When an autopsy is requested/required, SAVAHCS will arrange for transport of the decedent to the contractor's facility for performance of this service. The performance of this service will be done within two (2) working days of receipt of the body by a licensed and privileged practitioner. Upon completion of the service, the decedent remains will be released and transported to the funeral home the family has designated. Performance Monitoring: The contract holder shall be College of American Pathologists (CAP) and TJC accredited. The Contractor shall maintain accurate patient identification, provide testing as requested, and accurately transmit results. Compliance is monitored by incident and error tracking as part of the SAVAHCS Laboratory regular QA process. Preliminary anatomic diagnoses shall be faxed to SAVAHCS within two (2) working days of the receipt of the body. Exceptions to this requirement are the established contractor holidays that does not coincide with Federal holidays. Greater than 50% of the non-complicated cases shall be completed and reports received within thirty (30) business days. 100% of final reports shall be received by SAVAHCS within sixty (60) business days. Security Requirements: The contractor is subject to provision of the Privacy Act of 1974. The contractor shall keep confidential the contents of all medical documents entrusted to its care. All contractor personnel shall observe the requirements imposed on sensitive data and information by law, Federal requirements, VA statutes and policy, VHA policy and guidelines and the associated requirements to ensure appropriate screening of all personnel. Privacy Awareness Training: The contractor's staff and their subcontractor staff assigned to work under the agreement are required to receive annual training on patient privacy as established by HIPAA statutes. Training must meet VHA's and the Department of Health and Human Services Standards for Privacy of Individually-identifiable health information.
