- The Raymond G. Murphy New Mexico Veterans Affairs Health Care System hereinafter (NMVAHCS), located in Albuquerque, New Mexico is announcing the requirement for Base Year and Four Option Years Contract from businesses that are able to provide Contracted Community Based Outreach Clinical (CCBOC) Services. The Contractor shall provide continuous delivery of primary and preventive medical care services, including primary mental health services, for eligible Veterans enrolled with the NMVAHCS at the Contractor's clinic facilities that are to be located in the following locations: Las Vegas, NM / Espanola, NM and Taos, NM, for all assigned patients, in accordance with the terms and conditions stated herein, on behalf of the NMVAHCS, 1501 San Pedro Drive SE, Albuquerque, New Mexico, 87108. The Contractor's clinic sites shall located within each city limits in New Mexico. Contract is to be one contract with either one (1) award for all three (3) locations or multiple awards to different contractors. Contract will be for a Firm Fixed-Price indefinite quantity. SOURCES SOUGHT ONLY. THIS SOURCES SOUGHT NOTICE IS NOT A REQUEST FOR FORMAL PROPOSAL OR QUOTES. NO FORMAL SOLICITATION DOCUMENT EXISTS AT THIS TIME. This announcement is IAW FAR 5.203 as a Request for Information (RFI) ONLY; it is not a set-aside for Service Disabled Veteran Owned Small Businesses IAW FAR 19.14. Please respond to the FedBizOpps Sources Sought Notice and submit to this office by email not later than closing date on RFI announcement: 07/31/15 your companies interest in one (1), two (2) or all three (3) locations.
