July 08, 2015 MEMORANDUM FOR: Service-Disabled Veteran-Owned Small Businesses (SDVOSB) FROM:Orlando Veterans Affairs Medical Center Contracting Office (Room 210) 2500 Lake Mont Avenue Orlando, FL 32814-6102 SUBJECT: Request for Quotation (RFQ) VA248-15-Q-1231, Purchase and Installation of Medical Media Production Studio at Lake Nona, Florida 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and FAR 13.5, Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All responsible offers may submit a quotation that is timely received shall be considered. 2. This a RFQ for the purchase and installation of a Medical Media Production Studio (MMPS) which includes onsite training, maintenance and warranty services in support of fourteen independent and specialized work areas, inclusive of a three hundred and fifty person capacity auditorium. This requirement is solicited as a SDVOSB set-aside under Public Law 109-46, Veterans Benefits, Health Care, and Information Technology Act of 2006. North American Industry Classification System (NAICS) code 541512, Computer Systems Design Services, Size Standard $27.5M. 3. The Statement of Objectives (SOO) included as attachment A includes the Government's objectives for this procurement. Additionally, the following attachments are included and part of this solicitation: a.Applicable Clauses and Provisions, Attachment B b.Service Contract Act (SCA) Wage Determination, Attachment C c.Pricing Information Sheet, Attachment D d.Quotation Submission Instructions and Evaluation Criterion, Attachment E1 thru E2 e.Suggested Materials and Equipment List, Attachment F f.Facility Drawings, Attachments G1 thru G5 g.Past Performance Information Questionnaire (PPIQ), Attachment H h.Site Visit Information Sheet, Attachment I 4. By submission of a quotation in response to this RFQ, the Offeror certifies their adherence to all RFQ requirements, including but not limited to the Center for Verification and Evaluation (CVE) registration requirements, terms and conditions of the solicitation, and representations and certifications. Offerors shall clearly identify and provide sufficient rationale detail to justify any exception to the RFQ and/or its related documents. 5. The required delivery date for this RFQ is: 120 days, After Receipt of Order (ARO) 6. The point of contact for this RFQ is Ms. Rosa I. Rivera, Contract Officer, Lake Nona Activation Team. Funds are not presently available for this contract. This RFQ is not an authorization to begin performance and in no way obligates the Government for any costs incurred by the contractor in regards to this procurement. Offerors shall honor quote no less than 90 days after quote award. The Government reserves the right to cancel, this solicitation, either before or after the stated closing date for receipt of offers. In the event of a RFQ cancellation, the Government has no obligation to reimburse any potential Offeror for any costs incurred. 8. Written and electronic offers must be submitted in accordance with the RFQ submission instructions, on or before, August 7, 2015, no later than 11 a.m. Eastern Standard Time (EST). Rosa I.
