Description

This amendment is to confirm the SF330 due date and time. The SF330's due date and time is August 3rd, 2015 by 12:00 pm central time. This amendment is also to confirm that evaluation factor number five is only for firms that meet the required 150 mile radius of the York Medical Center. The firms that do not meet the 150 mile radius will not be evaluated. The prime contractors main office or branch office must be in the 150 mile radius and not an office from a subcontractor. In order to be considered for evaluation under this solicitation, please provide proof of office; to include a minimum of one (1) employee physically located at that office, performing design work for this project (if selected) and paying some form of rent for the shared space. Confirm address, phone number and name of employee in charge at satellite facility and landlord/owner of the building.
