- COMBINED SYNOPSIS/SOLICIATION VA246-15-Q-0990 DIOPSYS NOVA VISION TESTING SYSTEM (I)This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA246-15-Q-0990. (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. (IV)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (V)The Network Contracting Office 6-SAO EAST intends to award a firm-fixed price contract for Diopsys NOVA VEP Vision Testing System and supplies for the Fayetteville HCC, 7300 S Raeford Rd, Fayetteville, NC 28304-6162. (VI)The North American Industrial Classification System (NAICS) code for this procurement is 339115 with a business size standard of 500 employees. This procurement is unrestricted to and for all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. (VII)Questions concerning this solicitation shall be addressed to Tracy Hudson, Contracting Officer, and emailed to tracy.hudson3@va.gov. All questions or inquires must be submitted no later than 4:00 PM Eastern Standard time on Friday 10, 2015. No phone calls will be accepted. (VIII)The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price and meets the requirements of the SOW. Once inspected and accepted the VA will deliver to the Eye Clinic. At which point, the installation of hardware must be closely coordinated with the Eye Clinic in order to avoid service disruption to the patients. Optimal installation time is outside of normal clinic hours held 7:30a.m. - 4:30p.m., Monday through Friday. Following successful installation, all machines must perform all aspects of software functions. A confirmation that the machine functions accordingly will also include verification that all images are accessible from all machines. The manufacturer will provide a document that functionality has been confirmed. Said document must be signed by Biomedical Equipment Support Specialist, whose name will be forwarded upon notice of award, for validation. Training - Shall be provided by the manufacturer/vendor In-service training will be provided to pertinent Eye Clinic staff following validation functionality. Artifact detection aids in noise cancellation and accurate data acquisition/analysis. "Data display allows automatic cursor placement which gives the clinician the best fit approximation for the N75 and P100 cursor locations. Adjustable cursor locations for precise numerical data analysis. "Built in Wi-Fi "Point of care mobile cart for easy maneuverability between examination rooms. "Flat panel operator's monitor is adjustable in the up or down position to accommodate operators' size, or for standing or sitting. Monitor rotates up to 180 degrees.
