LOCAL STOCK NUMBER: LABOR 16.00HR____________________________________ GRAND TOTAL__________________ (vi)Performance Specifications: See salient characteristics for the requested items below: STATEMENT OF WORK Flooring Purchase and Installation for: Denver GRECC Research Program 7 July, 2015 554-15-3-876-0028 1.Title of Project : Flooring purchase and installation 2.Purpose of project: Purchase of floor covering to the GRECC (Geriatric) Research room to be used for the housing of various gym and stress equipment to be utilized in the study (Sq Ft. = 3,021) 3.Background: The Denver VA is starting a new Geriatric Research program, and is setting up an area to be designed to the specific type of research. 4.Tasks and Associated Deliverables: The contractor shall provide the flooring and then the installation of the flooring to the assigned research room. This will include removing all objects in the designated room before installation, and then replace of objects moved after installation of the flooring is completed. "The equipment to be moved/replaced includes a full line of Cybex resistance training equipment, treadmills, elliptical' s, cycle ergometers, and other miscellaneous exercise equipment (See pictures below). "The carpet will not be removed. Rubber will be laid over. "A durable rubberized floor will be needed to support the equipment being used, as well as preventing damage to the permanent flooring already in place. "The carpet is squares that are glued to the floor - low nap "We need an interlock system that provides a secure and nearly seamless fit - cannot have tripping hazards Veterans Affairs Department of Memorandum Date:July 08, 2015 From:Network Contracting Office 19 Subj:MFR Salient Characteristics | Rubber Tiles for GRECC (Geriatric) Research room To:Prospective Vendor 1.Brand Name Manufacturer: Humane. 2.Equipment/Services Requested: (See attached SOW) 3.Salient Characteristics: Below is a list of salient characteristics IAW FAR 52.211-6 Brand Name or Equal -- all "equal" products of the brand name manufacturer must meet the salient physical, functional, or performance characteristic specified in this solicitation. BRAND NAME ITEMS REQUESTED: 1.(See attached list) EQUAL ITEM SPECIFIC REQUIREMENTS: oHeavy duty commercial grade rubber flooring oFlooring must be ½ inch thick with interlocking individual pieces oMain portion of flooring is requested to be 4 foot x 4 foot interlocking pieces. Any other sizes must be submitted to contracting and evaluated by the Project Manager at the facility for technical acceptability. oFlooring must be easily cleaned with a dry mop because it will be installed over carpet. oMust have beveled edges for transitions between the rubber flooring and carpeted areas. oRubber must be non-absorbent with no curling. oTiles must be black in finish. oIncludes delivery, installation and labor; including moving and replacing existing equipment within the space before and after flooring installation.
