This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-14-Q-0763 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. The associated NAICS code is 339112 Surgical and Medical Instrument Manufacturing and small business size standard is 500 employees. The requirement is for 2 EA FIS-001: FLEXIBLE INSPECTION SCOPE or equal and 2 EA 100657: FLEX ARM FOR USE WITH FLEXIBLE INSPECTION SCOPE or equal. See below for description and requirements. Delivery location is the Louis Stoke Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, Ohio 44016-1702. Contract Title FLEXIBLE ENDOSCOPE for sterile processing 2. Scope The Scopes will be used to inspect the inside diameter of suctions, and lumened devices to ensure the absence of bioburden within the instrument. Residual bioburden can cause potential harm and jeopardize patient safety, this will also allow us to ensure ISO 9001 quality initiatives. 3. Specifications 1) To be used to visually inspect the cleanliness of medical devices MUST be capable of inspecting Lumened Instruments. 2) Must contain or combine with a camera in order to capture and document internal conditions of lumens 3) Insertion tube of the Inspection Scope shall have a flexible 50 cm or longer shaft and will have graduated markings every 10 cm. The diameter of the insertion tube of the inspection scope shall not be larger than 3mm in diameter 4) Must contain LED illumination 5) Must be non-fiber optic in order to be inserted into small lumens 6) Inspection scope must be able to be operated via controls on the handle, attached camera, or by capturing the image either on the attached computer or monitor screen. 7) Must provide both high resolution still and high resolution video camera capability with file storage. 8) Must have Indications for Use (IFU) for proper cleaning and disinfection of Inspection Scope. 9) Must be functional in a sterile processing environment. 10) The contractor shall provide education and training on the use and set up of the Flexible Inspection Scopes via In-service. 11) The Contractor shall provide detailed cleaning instructions and instruction manuals for all Flexible Inspection Scopes and related equipment to Sterile Processing Service. Evaluation and Award This acquisition will be conducted under FAR Part 13 and the evaluation and award will be made the offer that represents the best value to the Government. In addition to price, the following factors will be considered: technically meeting the requirement and the delivery/installation availability. The Government intends to make an award based on the initial quotations therefore offers should ensure that complete information and best offer is provided with initial submission. In addition to completing the Price/Cost Schedule offers must submit specification sheets that provide sufficient detail to allow for a technical evaluation, and any brochure or catalog information that will accurately depict the offered product and proposed delivery and installation date.
