- Attachment J.1 Past Performance Questionnaire Attachment J.2 Cost Proposal Template RFP DTNH2215R00056 The Scope of Work for this project consists of the design, test, administration, data analysis and reporting of a survey addressing drinking, drug-use and driving among the driving age public. The survey shall be a nationally representative sample of U.S. citizens 18 years and older and shall capture information on drivers' attitudes, behaviors and awareness of alcohol-involved and drug-involved driving. The sample size of the survey will be approximately 7,000 and produce nationally representative estimates of persons 18 and older. The survey shall be administered in English and Spanish. Conducting the survey requires approval from the Office of Management and Budget (OMB), and an Institutional Review Board (IRB). It also must meet Federal privacy regulations. This project shall follow a probability-based approach that delivers the questionnaire over the Internet and U.S. mail. The survey has been previously conducted using telephone surveys, with random digit dialing applied to a sampling frame of telephone exchanges. However, deteriorating response rates in telephone surveys and evolving technology within the survey environment require a new way to collect survey data. NHTSA intends to conduct the survey in an approach that emphasizes Web-based administration but includes a printed-on-paper mode through the U.S. mail. The methodology shall follow address-based sampling, U.S. mail contact, and random selection of a single respondent within households. The Contractor shall work with NHTSA to construct the questionnaire. The Contractor shall host the Web survey, administer the survey over the Internet and U.S. mail, and maintain and secure survey data during data collection. The Contractor shall also analyze the date, and prepare a methodology and results report. The objectives of the project include the following. To survey a nationally representative sample of drivers on their attitudes and behavior on drinking, drug use (including over-the-counter, prescription, and illegal drugs) and alcohol- and drug-positive/impaired driving; To assess the public's awareness, acceptance and opinions of laws on alcohol and drugged driving; To obtain information helpful to the monitoring of progress in impaired driving safety programs and reveal areas that require further attention; To add to the knowledge base supporting NHTSA's responsibilities for providing a sound scientific basis for the development of countermeasure programs; and, To obtain information, for the first time in this survey series, on drugs other than alcohol. The questionnaire and the methodology shall build upon the following sources (and possibly others that are not listed below). The eight previous versions of the National Survey of Drinking and Driving Attitudes and Behaviors, administered in 1991, [1], 1993, 2, 1995, [2], 1997, [3], 1999, [4], 2001, [5], 2004, [6],and 2008, [7]. The most relevant version for this project is the 2008 survey. The 2013-2014 National Roadside Survey, [8] a NHTSA project that collected information from drivers on their alcohol and drug use habits, as well as breath, oral fluid, and blood samples to measure alcohol and drug concentration. NHTSA's Drug and Alcohol Crash Risk: A Case-Control Study. [9] This study included measures of alcohol and drug use from crash-involved drivers and non-crash-involved drivers through questionnaires and biological samples (breath, oral fluid, and blood).
