Description
REQUIREMENT: 1.The Robley Rex VA Medical Center (VAMC) in Louisville, Kentucky, Social Work Service, has a requirement for non-personal services. The VAMC is currently in need of contract Licensed Clinical Social Worker (LCSW) services in the Emergency Department. Candidates will work on an intermittent (PRN) basis including on-call for weeknights, weekends, and holidays; and call back for evaluations. Contractor's employees shall be available at all times when assigned to work and shall respond by telephone within 20 minutes; and shall respond on-site at the Louisville VAMC within 90 minutes. 2.Contractor shall be required to maintain a pool of credentialed, badged, and trained LCSW's to support this requirement. Contractor's employees are estimated to be needed for on-site assessments approximately 750 hours per year. DUTIES: 1.Contractor's employees shall respond to the Louisville VAMC Emergency Department to provide psychiatric and medical emergency services as a licensed independent mental health provider. The requirement is to work nontraditional hours, including early mornings, nights, weekends, and holidays. S/he may be the only Social Worker on duty and must function at a highly independent, advanced level. The Contractor's employee also covers social work needs for Veteran's admitted to the medical acute care/critical care units during the evening and weekend hours. Coverage includes completing the "Social Work Admission Assessment" and providing patient assessments for any emergent issues that arise after hours. The Contractor's employee shall serves a patient population with highly complex medical and mental health issues. The Contractor's employee shall have experience that demonstrates possession of advanced practice skills and judgment and at least two years in an area of specialized social work practice. The Contractor's employee must be licensed at the advanced practice level. Candidates are required to have specialized knowledge of psychiatric emergency diagnosis, intervention, and treatment. Contractor's employees shall also be skilled in the areas of post-traumatic stress disorder, domestic violence, sexual assault, child, elder, and dependence abuse, substance abuse, grief and bereavement, and end of life issues. Functions of the position include: performing specialized assessment of complex physical and mental illness for patients in the emergency department with psychiatric emergencies. Will also complete initial psychosocial assessments on Veteran's admitted to Acute Care and Mental Health during the weekend along with assisting any veterans/families who have not yet been assigned to a Social Worker. Services are primarily provided in the emergency department with very little supervisory control. The Contractor's employee is typically the only Social Worker on duty during early mornings, evenings, weekends, and holidays. Contractor's employees must demonstrate advanced clinical skills and be able to function independently. Contractor's employees shall incorporate complex multiple causation in differential diagnosis of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Specific tasks may include: evaluation, DSM diagnosis, and intervention for psychiatric emergencies in the emergency department; psychosocial assessment, treatment planning, and coordination of care for patients that require inpatient mental health placement; consultation with other emergency services; and end of life counseling. Provide services on an outpatient basis. 2.Contractor's employees shall perform a psychiatric assessment, recommend disposition for admission or release, and document the encounter in the VA's electronic medical record per VA guidelines. If Contractor's employee recommends admission for inpatient psychiatric treatment, the employee shall provide a verbal report to the psychiatric resident at Emergency Psychiatric Services (EPS) at the University of Louisville requesting admission orders. If Contractor's employee recommends release from the Emergency Department, the employee shall contact the psychiatrist on-call to provide a verbal report. The psychiatrist on-call will make the final decision regarding whether to release the Veteran. For all Veterans seen in the Emergency Department, the applicable outpatient psychiatric providers will be additional signers on the consult. 3.Mental health assessment documentation must include: "Chief Complaint "History of present illness "Mental status exam "Evaluation of suicidality and homicidality "Review of Vital Signs "Determine and annotate DMS-V diagnosis "Make treatment recommendations "Prepare a medication list "Recent lab values and imaging must be included as applicable "Completion of CPRS medical record concurrent with assessment/evaluation TOUR OF DUTY AND RESPONSE: 1.Irregular tour of duty that will vary based upon need to include weeknights from 4:30 pm to 8:00 am, weekends from 8:00 am on Saturday to 8:00 am on Monday, and holidays from 8:00 am on the holiday until 8:00 am the following day. Contractor's employees are expected to respond to the VAMC approximately 750 hours a year, and perform approximately 6,736 hours of on-call support. 2.Response Requirements: Contractor's employees shall be available at all times while on-call via a pager or phone and shall respond telephonically within 20 minutes of receiving a page or call from the Louisville VAMC Emergency Department attending physician. The Contractor's employee must be on-site at the Louisville VAMC Emergency Department within 90 minutes of answering a page or call to initiate/conduct patient evaluations. QUALIFICATIONS REQUIRED: 1.The basic requirements for Contractor's employees include the following: a. Citizenship. Be a citizen of the United States. b. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. c. Licensure. Persons must have a full and unrestricted license, and shall not have had their license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or have been denied a license upon application. The employees must be licensed or certified by a state to independently practice social work at the master's degree level (shall meet all requirements to be considered a licensed independent provider). Current state requirements may be found on the OHRM website. d. English Language Proficiency. Social workers must be proficient in spoken and written English. e. BLS Trained. The Contractor's employees shall have a current BLS certification, and shall maintain BLS certification at all times s/he works under the contract. f. Standard Personnel Testing (PPD). Contractor's employees shall provide proof of the following tests to the CO/COR for each employee/provider prior to the first duty shift, and all tests shall be current/performed within the prior 12 month period: "Tuberculosis Testing: Proof of a negative reaction to PPD testing for all employees/providers. A negative chest radiographic report for active tuberculosis shall be provided in cases of positive PPD results. The PPD test shall be repeated annually. "Rubella Testing: Proof of immunization for all contract employees/providers for measles, mumps, rubella, or a rubella titer of 1.8 or greater. If the titer is less than 1.8, a rubella immunization shall be administered with follow-up documentation to the COR. "OSHA Regulation Concerning Occupational Exposure to Blood Borne Pathogens: Provide generic self-study training for all contract employees/providers; provide their own Hepatitis B vaccination series at no cost to the VA if they elect to receive it; Contractor shall maintain an exposure determination and control plan; Contractor shall maintain required records; and Contractor shall ensure that proper follow-up evaluation is provided following an exposure incident. The VAMC shall notify the Contractor of any significant communicable disease exposures as appropriate. TECHNICAL PROFICIENCY: Contractor's employees shall be technically proficient in the skills necessary to fulfill the Government's requirements including the ability to speak, to understand, and to read and write English fluently. Contractor's employees shall provide documents upon request of the Contracting Officer/Contracting Officer's Representative to verify current and ongoing competency, skills, certification, and licensure related to the provision of care, treatment and/or services performed. Contractor's employees shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all employees/providers. TRAINING REQUIREMENT: Contractor's employees shall complete all VA required training including on-site training. In-person training at the Louisville VAMC shall be paid at the rate listed in the price schedule. On-site training shall include, but not be limited to, training on proper use of the VA medical record system (CPRS), introduction to VA policies and procedures, and VA orientation. Contractor's employees shall also be required to complete certain web-based training programs such as infection control, HIPAA and security training, hand washing and hygiene, etc. PERIOD OF PERFORMANCE: The Robley Rex VAMC expects to enter into a contract with a provider for a period of one-year from the effective date of the contract, with the possibility of extending the contract for a period of up to six months at the same prices/rates. CREDENTIALING AND BACKGROUND CHECK: Contractor's employees shall be required to timely complete respond and complete the credentialing process immediately after notice of award. Additionally, Contractor's employees shall timely provide fingerprints and submit documentation for a background check immediately after notice of award. Contractor's employees shall comply with all credentialing requirements in order to maintain current credentialing. PRICE SCHEDULE: CLINDESCRIPTIONEST. All prices are for the full period of performance and any contract extension periods MISCELLANEOUS: 1.The Federal Tort Claims Act does not cover Contractor or Contractor's employees/providers. Contractor shall be responsible to maintain and provide evidence of liability and malpractice insurance covering all Contractor's employees/providers. If Contractor or Contractor's employee has been identified as a provider in a tort claim, the Contractor shall be responsible for notifing their legal counsel and/or insurance carrier. Any settlement or judgment arising from Contractor or Contractor's employee/provider action or non-action shall be the responsibility of the Contractor and/or Contractor's insurance carrier. 2.The Contractor shall be required to have a contingency staffing plan in place to ensure continuity of care as continuity of care is an essential part of the VA's services. The contingency staffing plan shall ensure the Contractor is able to continue performance of services in the event an employee is unable or unwilling to continue performance. 3.The Contractor is responsible to ensure that all on-call services are provided in accordance with contract terms, that all responses are made timely in accordance with contract terms, and that Contractor's employees comply with all credentialing, licensing, and training requirements. 4.The Contractor shall be furnishing health care services and shall be considered a health care provider for the purposes of this Contract. Therefore, Contractor shall be responsible for ensuring that Contractor and all Contractor's employees comply with all Privacy standards and laws, including HIPAA. 5.Contractor shall supervise all Contractor's employees/providers and the VA shall not provide any supervision of Contractor's employees/providers. The VA shall utilize a Quality Assurance Surveillance Plan (QASP) to monitor Contractor's performance and compliance with contract terms.
