This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. II. NHLBI-CSB-HV-2015-134-JS is issued as a request for proposal (RFP). III. The solicitation document and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. IV. The associated NAICS code for this requirement is 541380, with a small business size standard of $15M. The acquisition is being conducted in accordance with the procedures of FAR Part 12 and Part 15. V. Contract line item(s): TBD VI. Please see Attachment 4 - Statement of Work for a description of the four (4) separate topic areas where each offeror may submit to one (1) or more areas. Each topic areas will be evaluated as a standalone proposal and not as a whole. The government has the right to determine which topic areas an offeror proposes will be awarded. Selected offerors will received 1 contract regardless of the number of topic areas selected to be awarded from their proposal. The government has the right to make multiple awards for each topic area. VII. The period of performance of this requirement will be one year. VIII. The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the dis¬putes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provi¬sion of law. XIV. All offerors shall electronically submit, a proposal addressing the attached SOW to Jennifer Swift, Contracting Officer at Jennifer.Swift@nih.gov by 3:00pm Eastern Standard Time (EST) Friday, July 31, 2015: The proposal must reference the RFP No. NHLBI-CSB-HV-2015-134-JS. All responsible offerors may submit a proposal, which if timely received, shall be considered by the agency. Should an offeror have questions regarding this solicitation, in order to provide a quick response, the contracting officer asks that ALL questions, be carefully composed and recommends the offeror use best efforts to limit their questions submitted to ten (10). It is also asked that only one email is received per offeror. Questions in response to this solicitation are due no later than 12pm (Noon) EST Monday, July 20, 2015 with a response date on or before Friday, July 24, 2015. Faxed proposals will NOT be accepted. Offerors' proposals shall not be deemed received by the Government until the proposal is received by the e-mail address inbox set forth above.
