Description
This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by the VA Hospital in Buffalo, NY for the purpose of collecting information about the Lab Courier Services. The VA is seeking to award a base plus 4 option years contract for Lab Courier Services as described in the attached SOW. (e) Contractors that are able to meet the requirements in the attachment are encouraged to email full information to Anthony Murray at Anthony.Murray2@va.gov. - Contractors shall identify the NAICS code for the product being offered as well as their size status under said NAICS. - Contractors shall also identify any Federal Supply Schedules that may carry the desired product. - Contractors shall include any relevant comments about the Attachment(s). VA Western New York Healthcare System Statement of Work Local Courier Service 1.Background: VA Western New York Healthcare System, Buffalo NY 14215 proposes to award a fixed price contract to provide transport services for patient samples that require testing at a local facility and for the transportation of blood products for transfusion between the VA, local facilities and the American Red Cross Reference Laboratory in W. Henrietta, NY. 2.Period of Performance: Base Period: October 1, 2015 - September 30, 2016 Option Year #1: October 1, 2016 - September 30, 2017 Option Year #2: October 1, 2017 - September 30, 2018 Option Year #3: October 1, 2018 - September 30, 2019 Option Year #4: October 1, 2019 - September 30, 2020 3.Scope: a.The contractor must provide the pick-up and delivery of blood specimens and blood products between the VA Western New York Healthcare System and local hospitals and Reference Labs within 1.5 hours from the time the request for services is generated, 24 hours/day; 7 days/week, 365 days a year, including holidays. b.The contractor must have the ability to supply an occasional STAT delivery request entailing delivery times of less than 1 hour from time of request. c.Drivers must wear uniforms and/or identification badges. d.Orders for transportation services will be requested by a telephone call to the contractor by the Laboratory staff. The originating and destination points will be identified at the time the request for service is made along with any STAT delivery request. e.Invoices should be generated on a monthly basis and sent electronically to Austin Fiscal for payment. f. Invoices must include: 1. Purchase order # 2. pick-up and delivery locations 3. name of tech calling for service 4. date/time of delivery 5. Name of person signing for delivery. g.Contractor must supply service to destinations including, but not limited to, the following Pick up/Delivery Addresses AMERICAN RED CROSS REFERENCE LAB 825 JOHN ST West Henrietta, NY 14586 IMMCO 640 ELLICOTT ST BUFFALO, NY 14203 BUFFALO GENERAL HOSPITAL 100 HIGH ST BUFFALO, NY 14203 ERIE COUNTY MEDICAL CENTER 462 GRIDER ST BUFFALO, NY 14215 CENTER FOR LAB MEDCINE 115 FLINT RD AMHERST, NY SISTERS OF CHARITY HOSPITAL 2157 MAIN ST BUFFALO, NY 14214 CHILDRENS HOSPITAL OF BUFFALO 219 BRYANT ST BUFFALO, NY 14222 SOUTH BUFFALO MERCY HOSPITAL 565 ABBOTT ROAD BUFFALO, NY 14220 VA WESTERN NY HEALTHCARE 3495 BAILEY AVE BUFFALO, NY 14215 4.Qualifications: a.Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. b.The contractor has to be Department Of Transportation certified. c.The Contractor must have documented Blood Borne Pathogen Training for its employees. 5.Quantities: a.This contract is written on a per case basis. The VA Western New York Healthcare System will be charged a per route rate for each delivery. Please complete Pricing Schedule for the Base and option years. TermEstimated Quantity per Month Base Year30 Option Year #130 Option Year #230 Option Year #330 Option Year #430 6.Contract performance monitoring a.Invoices will be certified by the COR (Contracting Officer's Representative) prior to payment. This will include a cross check against services invoiced and services received. b.A log will be maintained in the contract file relative to problems or complaints associated with this contract. This log will document that mutually agreement resolution is reached regarding each issue in a timely manner. 7.Subcontractor provided services: All individuals who provide services under this contract and are not employees of the Contractor will be regarded as subcontractors. The Contractor shall be responsible and accountable for the quality of care delivered by any and all of its subcontractors. The Contractor shall hold the subcontractor accountable for the requirements of this contract.
