- RFQ Amendment HT9402-15-Q-0008-0001 Ensure Block 15A., B. and C. are filled out, scanned and emailed with Quote. **AMENDMENT 0001*** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. The solicitation number is HT9402-15-Q-0008-0001 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-82, June 8, 2015. The associated North American Industrial Classification System (NAICS) code for this procurement is 611430, Professional and Management Development Training, with a small business size standard of $10,000,000.00. This RFQ is unrestricted, and all responsible sources may submit a quote, which, if received timely, will be considered. Offerors shall email any questions to melissa.a.booker5.ctr@mail.mil as early as possible to permit DHA to respond. QUESTIONS RECEIVED BY PHONE WILL NOT BE ADDRESSED. Signed and dated quotations shall be received by 2:00 PM Mountain Daylight Time, Friday, July 17, 2015.
