SAM.gov registration step by step guide Tax Exempt Letter Vendor Packet EEO/AA Non-Segregated Davis Bacon "THIS IS A SUBCONTRACTING OPPORTUNITY" Emergency /Exit Lighting Deficiency ID# 104290 The Brooklyn Job Corps Academy is soliciting bids for someone to correct deficiency findings with our exit signs located in our corridor. The Brooklyn Job Corps Academy is a federally funded vocational and educational training program for youth. The program is open year round with approximately 205 trainees. The Brooklyn Job Corps is a satellite center of the South Bronx Job Corps and it is operated by ResCare, Inc. for the US Department of Labor. SCOPE OF WORK Provide two new single sided, ceiling-mounted light emitting diode (LED) type exit sign above the exit door in the corridor by GED room (240) and one same corridor by reading room (237). The new exit signs shall comply with NEC and NFPA 101 code requirements and have long-life batteries backup, test switches and indicator lights. Connect to the nearest lighting circuit serving the same respective area; connect ahead of any switch. For estimating, assume the installation of two new single-sided, ceiling-mounted, light emitting diode (LED) type exit signs and 40 LF of 3 #12 wire in 1/2" EMT. REQUIREMENTS • Must be in compliance with all Local, State and Federal Guidelines and Regulations. • Work to be accomplished in a safe and efficient manner with minimal or no disruptions or endangerments to trainees and/or staff. • Must provide a copy of current licensure and insurance. • Must be registered on the SAM.gov Website • Must complete Vendor Packet. Which consist of the following items: • New Vendor Form. Fill out entire form accordingly. DUN# SECTION IS A PRIORITY. • W-9 Form • Non-Conflict of Interest • Terms & Conditions and • Anti-Bribery Form-Sign Approval box. ACKNOWLEDGEMENT OF ACCEPTANCE • Contractor must acknowledge by signature that they meet all Federal, State, Local and OSHA regulations and that all specifications terms and conditions set forth are acceptable. • Bidders must submit their proposals via email, fax or mail in a sealed envelope by 5pm close of business on July 21, 2015 to the attention of Barbara Kimbrough, Purchasing Agent. We have a deadline of July 31, 2015 for the job to be completed. This is a prevailing wage job. The following documents Davis Bacon, Certification of Non-Segregated Facilities and Equal Opportunity and Affirmative Action Requirement will be incorporated and made part of agreement.
