Description

This is a synopsis for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. NAICS: 339113 This requirement is being issued as 70% Small Business Set-Aside. SBA Size Standard: 500 employees The Department of Veterans Affairs (VA) has an ongoing program to furnish artificial limbs, orthotics and related services to those veterans that require them and are eligible for assistance. It is VA's intention to solicit proposals and award multiple contracts on a competitive basis to preferred providers within the VA Desert Pacific Healthcare Network that includes the following VA Medical Centers: Los Angeles, CA; Long Beach, CA; Loma Linda, CA; San Diego, CA; Las Vegas, NV. Veteran beneficiaries have the right to select the artificial limb contractor of their choice from any VA approved contractor. VA personnel shall not direct, guide, or prompt a Veteran beneficiary to a specific contractor except when a physician determines that it is a medical necessity. To assist in the selection of their Prosthetist, all eligible veteran beneficiaries shall be given a current list of the contracted vendors located within the area of jurisdiction a specific contract is awarded for. It is the VA's policy to permit all service-connected and non-service-connected amputees who have continuing eligibility and who have established ties with an existing Prosthetist to continue to use that Prosthetist provided that Prosthetist is willing to accept the current contract provider terms for the applicable contract area. The solicitation will be issued on/about August 1, 2015 and is projected to close on/about August 26, 2015 at 3 PM PST. The VA will issue the solicitation electronically and accessed at the Federal Business Opportunities website: www.fbo.gov. It is the contractor's responsibility to monitor the site for any documentation, information, etc. regarding this requirement. System for Award Management (SAM): Pursuant to FAR Clause 52.212-4, para (t), System for Award Management (SAM) after initial registration, the contractor is required to update registration data as changes occur and must re-register annually to ensure all data remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included herein and will be cause for termination of the contract at such time noncompliance is discovered. Refer to cited Clause for more details. Required Registration with Contractor Performance Assessment Reporting System (CPARS): As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all contracts that exceed $100,000, and shares those evaluations with other Federal Government contract specialists and procurement officials. The FAR requires that that the contractor is provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement, VA uses an online database, the Contractor Performance System (CPARS), which is maintained by the National Institutes of Health (NIH). The CPARS database information is shared with the Past Performance Information Retrieval System (PPIRS) database, which is available to all Federal agencies. You are required to register with the CPARS database at the following web address: https://cpars.gov Further, your registered representative data, as well as any other information in your registration data, must be kept current. Performance evaluations entered into CPARS by the contracting officer will be assigned to the contractor's registered representative for comment. Failure to comment and reassign the report back to the contracting officer, or to maintain current registration, will result in the Government's evaluation being entered in the database with a statement that you have failed to respond.
